Risk Screening Team
Student’s behavior becomes “disturbing”. *Adults become alarmed*
Administration is considering a SSA Risk Assessment (when the student is NOT transported to Juvenile)
STEP 1

Risk Screening Team is formed. Referral made on RST Form #1

RST review screening report, determine level of Intervention
NO FURTHER ACTION
No Risk factors are present, or only one or two
potential violence risk factors are present with no
evidence of imminent violent risk factors.
Develop, implement and monitor an Intervention
plan if warranted.
WAIT AND SEE
No imminent risk factors present but several potential risk
factors are present and problems could develop later to
warrant a comprehensive assessment. Develop, implement
and monitor an Intervention Plan. Monitor the evidence of
deterioration and recommend a comprehensive risk
assessment if concerns persist.
STEP 5
Recommend a comprehensive risk assessment if
concerns persist
One imminent risk factor present and several potential risk
factors are present and problems could develop later to
warrant a comprehensive assessment. Develop,
implement and monitor an Intervention Plan. Monitor the
student more closely for evidence of deterioration and
recommend a comprehensive risk assessment if concerns
persist.
Recommend a Comprehensive Risk Assessment
Multiple potential risk factors are present that suggest
serious and possible immediate risk or one or more
imminent risk factors are present. Refer the student for
a comprehensive risk assessment by SSA. Closely
monitor the student and devise a safety plan. A change
of placement pending the assessment may be
warranted.
