This “Contract for child care services-fee agreement” is made this ________ day of________________,

We agree to provide child care services for the above listed child in accordance with the mission,
philosophy and policies outlined in the center’s parent handbook. The child is enrolled in the

Classroom assignments are made based upon the age of the child in accordance with childcare
licensing regulations.
This contract for childcare services agreement will expire, when either the child changes rooms,
tuition amount changes, or the schedule for the child changes. The parents of the above named
child agree to update the contract for childcare service when these events occur.
Annual Fee:
An annual fee of $25.00 per family is charged to help us keep our environments, toys and
activities new and exciting.
Schedule Change Fee:
Any requests for changes to the enrollment schedule listed herein must be made in writing and
submitted to the Director two weeks prior to the requested change. This change may require an
execution of a new contract for childcare services-fee agreement and payment of any additional
registration fees and/or tuition increase. We reserve the right to deny any request for schedule
change for any reason within its sole discretion. We need to make sure we are within licensing
guideline ratio when making the decision. ** There is a $25.00 fee for each schedule change the
parent’s request.

By signing below, the parents/guardians of the above listed child hereby acknowledge that
I/we have read this contract for childcare services-fee agreement completely, that I/we have had
the opportunity to received complete written program information and discuss the information
contained herein with a representative of Right from the Start that our questions have been
answered fully and to our satisfaction and that we agree to abide by the conditions set forth
herein as well as the polices contained by reference from Right from the Start’s parent handbook.
Creative Wonders Childcare will reserve a place for your child with the contract, one week
deposit $______ and a one-time $25.00 non-refundable processing fee; future enrollments of
siblings require no processing fee.

This contract for childcare services agreement may be cancelled by the Parents/guardians with a
written two weeks’ notice. The written notice of cancellation must be submitted to the director of
Creative Wonders Childcare by parents/guardians.
This contract for childcare services agreement may be cancelled by Creative Wonders Childcare
at any time with or without notice, in its sole discretion. An unused tuition paid by the
Parents/guardians will be first applied to any unpaid account balance, if no balance the amount
will be refunded to parents within 60 days.
