NOMINATION GUIDE
ORILLIA MUSEUM OF ART & HISTORY
HISTORY AWARDS 2015
Nominations must be received at the Museum by-4:00 p.m. Monday, September 14, 2015
Conditions
Four Orillia Museum of Art & History Awards are offered bi-annually starting in 2015, for
historic research or heritage presentation in Orillia to the Orillia and area. Orillia and area
includes: the City of Orillia, the Townships of Ramara, Severn, and Oro-Medonte.
There will be up to four awards and up two recipients in each category.
Categories for Nomination
History Awards for Research, Publications and/or Education – there are two categories
in this section, as follows:
General History – recognizes someone whose scholarly research results in a publication that will
expand the permanent public record of history knowledge and/or someone whose communication
or teaching skills inspire greater public awareness of, and interest in history, over the last five
years.
Lifetime Achievement – recognizing someone who has contributed to Orillia and area history
over many years which represents a lasting, historic legacy.
History Awards for Preservation of Heritage
- there may be up to two categories as follows:
General Heritage Preservation - recognizes someone for compiling a significant group of
artifacts, and repairing or restoring them to represent the original conditions.
Historic Reconstruction – recognizes someone whose re-creation of a historic interior, structure
or landscape brings to life the material environment of an earlier time.
Judges Recognition – honourable mention to people who are not eligible for an award, but who
contribute over and above to promote/preserve Orillia and area history, i.e., employee of local institutions
who go above and beyond their line of duty. No prize. No award.

Preparing your Nomination Package(s): you may nominate multiple candidates for the Awards.










The candidate for an award may be a single person, or a group that has collaborated
Nominations are accepted for activities over the last five years or more
The candidate must be aware they are being nominated
Please ensure you identify which category you are nominating a candidate for (Mark the box
above for the category of the nomination.)
Give a clear overview – don’t assume everyone knows about the project or person
Please provide supporting materials, i.e., books, photographs, media coverage and at least two (2)
letters of support
Keep candidate’s nomination materials together in a single package, clearly labeled outside
Nomination packages will NOT be returned, so be sure to keep your originals – send copies only!
Please include a filled-in copy of this form with each candidate’s nomination package
A short-list of up to five (5) nominations will be selected in each category, of which an award
recipient will be selected
