This position will support the overall Coordinated Entry assessment and referral process for youth
experiencing a housing crisis in Dakota County.
Duties and Responsibilities:
 Manage Housing Crisis line calls for youth (18-24) experiencing a housing crisis in Dakota County
and respond to all calls within 24 hours
 Conduct the VI-SPDAT to determine eligibility and housing need
 Provide referrals to C.O.R.E. and other community resources to assist youth with systems
navigation, basic needs, and other resources and supports as needed
 Coordinate with Dakota County Supportive Housing Unit on managing referrals and youth on
waitlist
 Support youth on waitlist whenever possible with systems navigation and supportive services to
meet their needs
 Collaborate with other Dakota County human, social, and educational services to identify youth
who may be experiencing a housing crisis
 Participate in all activies related to Coordinated Entry in Dakota County
 Attend all relevant agency meetings and trainings
 Other duties as assigned.

professional interactions with clients, co-workers, and all parties involved
 Recommends changes when necessary for the betterment of the Agency
Education and Experience:
 BA degree in related field or 4 years of experience working with young people who have
experienced homelessness
 Experiencing supervising front end, critical intervention services for young people at-risk of or
experiencing homelessness
 Experience working with young people who have significant barriers to housing and stability
 Experience working with young people with disabilities
 HMIS
