On the pages below you will find 2 intake forms you can customize to meet your business needs. If you're going to keep them as Word Documents, add a header and footer with your biz logo and contact information to fancy it up.

If you don't want to use Word Documents and have to send them back and forth (which can get cumbersome), here are a couple of other options:

1. Set them up in Google Drive. Once you have them set up there, you can publish the forms on your website and your clients can access them from there.

2. Another option is to use a contact form on your website that you can direct clients to.

After they submit the form, set it up so it will be emailed directly to you.

Basic Client Intake Form

The first form is a basic one you can use to collect data from a prospective client. This type of intake form is a great way to get some background information about a possible client - find out more about their business, who their market is and what they are looking for help with.

If you are going to have a consult call with them, ask them to take a few minutes to fill out the form before your scheduled call. Having them do this beforehand helps you get a better feel for their business and what it is they need help with and then you can ask more in depth questions on your call.

Login Information Intake Form

The second intake form is one you can use to collect information from a client you will begin working with as their Google Hangouts VA. Customize it to meet the needs of each client you're working with.
