Proponent Mandatory Questionnaire
It is mandatory that the Proposal response to Appendix ‘D’ is provided directly onto the
Appendix ‘D’ form. Proponents are to provide their Proposal response after the word
“RESPONSE”. If no response is applicable, please indicate Not Applicable. The responses
provided will form part of the evaluation.
If the response is lengthy additional detailed information to the original response can be referred
to and provided as appendices for clarity and convenience.
Proponent(s) must not alter any portion of Appendix ‘D’, with the exception of adding the
information requested. To do so will invalidate the submission of its Proposal.
Company Profile, Experience, Qualifications

1. Describe your company’s primary business and its experience with the supply of Johnson
Controls Digital Control Parts and Service to DDC’s?

The City has identified a number of Parts it has purchased within the last two (2) years to
compare and evaluate each Proponent’s proposed individual product pricing and/or a pricing
discount structure. These Parts are not guaranteed and are for the purpose of evaluating
Proposal(s) only. ** Please note if and when the City orders Part(s) from the successful
Proponent that is awarded the contract, the City expects the same pricing structure used for the
Part(s) identified in this section.
Proponents are to provide unit price, unit purchase price, extended price for Part(s) set forth in
this section. All Part(s) are matched 100%. If the Proponent’s Part is different, than the City’s
specification for the Part, the Part is deemed not to be a match. If the Proponent cannot provide
a Part, indicate “X” for No Bid.



Unit Price means the unit price the Proponent pays for the Part(s).
Unit Purchase Price means the unit price the City pays which excludes freight and
mark-up.
Extended Price is calculated based on the Estimate Usage x the Unit Purchase Price =
Extended Purchase Price.

4. Provide a firm hourly charge out rate for service at the City’s facilities located in Nanaimo,
British Columbia. Hourly rates exclude HST. Hourly rates apply to on-site work only and
include labour, travel time, service vehicle charges, fuel surcharges, tools, insurances,
permits, licenses and all other requirements necessary to complete the services. Parts and
HST are excluded. Service may involve:
All hourly rates submitted to be firm for the initial two (2) year contract. The hours shown
below are estimated hours only and are for the purpose of evaluating Proposal(s) only.
Actual hours may vary. The City reserves the right to increase or decrease the scope of the
work and the quantities of hours indicated.
