Welcome and Important Information on Enrollment in UT-SPH (1st in a series)
Congratulations on being admitted to the UT School of Public Health! We are excited to
welcome you as a member of the entering class of Spring, 2012. In the coming weeks, you will
receive a series of emails containing important information to prepare you for registering and
enrollment this fall. Completion of these steps will enable you the best possible enrollment
process. Be sure to update your email and address with the Registrar’s Office if they change.
Confirm Your Attendance: If your plans to enroll for the Spring 2012, have changed, please
inform the Student Affairs Office by updating the Admission Response Form at:
https://sphuth.qualtrics.com/SE/?SID=SV_b9jDMRbi5BvjliI. If you wish to defer your
enrollment to a later date, you may make that request by contacting Dr. Mary Ann Smith,
Associate Dean for Student Affairs at Mary.A.Smith@uth.tmc.edu.

As a new student, you will be asked to identify yourself with your personal student
identification number, known as your A number. It is used to log in to UT Link, to register,
and for various identification purposes. If you have never been on UTLINK, your default PIN
is set as the last two digits of the year you were born and the two digit month you were born.
For example, July 1980 would be entered as 8007. You will be required to change your PIN the
first time you log in to UTLINK and at various times throughout the year.
