If granted, permission would be subject to the following conditions:
1. Reproduced material would need to be clearly identified with the name of the copyright holder, author(s)/editor(s),
year of publication, book/chapter/article title, and page number(s) where material appeared.
2. Permission would be granted for nonexclusive, one-time use only, as described in the request. Electronic or any
other usage would require a separate request.
3. Permission would be granted for use in the English language only. Translations of materials would require a separate
request.
4. Permission could only be granted if the material is original to an Oncology Nursing Society publication. If the
requested material has appeared in our publication with credit or acknowledgment to another source, it would be
the requestor’s responsibility to secure permission from that source, as well.
