Funding
Enquiries and Referral
Lifestyle Support partners with clients in receipt
of individualised funding packages and those in
Ageing, Disability and Home Care (ADHC) blockfunded arrangements.
If you would like to talk to someone from Lifestyle
Support or make a referral please contact us by
telephone, email or write to us at the addresses
below.
We can supply a package of support under the
National Disability Insurance Scheme (NDIS) or
Individually Funded Support packages. This
includes funded support services either agency
managed, self-managed or a combination of
both.
We will help people with disability gain
more choice and control over the
services they receive, and how these
services are provided.
For supported accommodation, clients pay a
percentage of rent and living expenses and a
service fee.
Accreditation
Lifestyle Support is pre-qualified by NSW Family
and Community Services, ADHC to provide
supported accommodation, and by the NSW
Lifetime Care and Support Authority and the
Victorian Transport and Accidents Commission as
an Attendant Care service provider.
Royal Rehab is not an emergency service.
In the event of an emergency, please call the Ambulance Service on 000.
We advise you to seek your local doctor for other medical issues.
Lifestyle Support is accredited with the Attendant
Care Industry Association, the National Standards
for Disability, and Australian Council on
Healthcare Standards.

Accommodation
and Support Services
Who we are
Lifestyle Support offers supported
accommodation, attendant care services
and specialised support programs to people
with a disability. We support a person to live
in a home of their choice, have the lifestyle
they choose and to be an active participant
in their chosen community and activities.
Who we support
We support people with acquired
disabilities (brain injury and spinal injury),
developmental disabilities (intellectual,
autism spectrum disorder, cerebral palsy)
and other degenerative neurological
disabilities. We also support people with
complex healthcare needs, co-existing
psychiatric disability or significant
behaviours of concern.
Where we are
Lifestyle Support services are available
throughout Sydney's metropolitan area and
Greater Sydney. Our head office is located
within the grounds of Royal Rehab at Ryde,
Sydney.
Accommodation Options
A mix of accommodation options are
available, ranging from services being
provided in a person’s own home,
independent living units/apartments, group
homes (up to six people) and cluster-type
residences for people with complex needs
within the grounds of Royal Rehab.
Support Services & Programs
Support provided depends on the assessed need
and the funding arrangement for each person.
Services range from occasional drop-in to
intensive 24/7 support.
Attendant Care Services and Support
Worker
Nursing and Healthcare Planning
A Nursing Service operates seven days a week and
supports people who may need nursing care or
health monitoring. Individualised healthcare
planning is also available to ensure heath support
needs are identified and managed and to ensure
the best outcome for a healthy lifestyle.
Attendant care services available include support
with:













Personal care
Meal preparation, cooking, eating and
drinking
Shopping
Making and attending appointments
Laundry, gardening and other household tasks
Taking prescribed medications
Recreation, leisure and community access
Accessing education and employment
Banking and personal administration
Decision-making and choice
Liaising with other support services and
community networks
Maintaining friendships and social networks
Accessing cultural and spiritual activities
Person Centred Planning
Skilled and qualified Person Centred Support
Planners are available to assist people to realise
and set their goals relating to any area of their life.
Positive Behaviour Support
Skilled Positive Behaviour Support Coordinators
are available to work in partnership with the
person and their support network to support them
with developing skills and contribute to improving
the person’s quality of life.
Recreation, Leisure and Community Access
A specialised Recreation Service is available to
assist in the planning and development of
recreation and leisure programs, to support skill
development, to facilitate community inclusion
and to assist in linking the person to peer support
and social networks.
Respite, Short-term & Emergency Support
Respite services and short-term stays may be
provided when vacancies occur or in the person’s
home. Emergency support will depend on funding,
assessment and capacity to respond at the time.
