On January 5th, Nottinghamshire County Council announced the award of their Obesity Prevention and
Weight Management Services to Everyone Health (part of SLM).
Who is Everyone Health?
At the core of Everyone Health is a highly skilled expert team with extensive experience in strategic and
operational management of health business at a national and European level. As key drivers of the
business, this team is passionate about quality, service effectiveness, and the application of evidence
based approaches to pragmatic and effective operational delivery.
Everyone Health offers a holistic approach to improving the health of local communities by delivering
integrated, dynamic and bespoke prevention and treatment solutions, applying a strong clinical
governance and quality assurance approach, in preparation for, and delivery of all services. In recognition
of this, we are a registered provider with the Care Quality Commission and the Health and Social Care
Information Centre, compliant with their Information Governance requirements.

Overview summary of our integrated service model: Our community-based, accessible service model is
designed in line with NICE guidance, SIGN and the evidence-base, delivering a model that is a
multicomponent intervention, including a multidisciplinary expert workforce providing comprehensive
assessment and interventions incorporating diet, physical activity and psychosocial issues to promote
sustainable behaviour change for children, young people and adults. Behaviour change strategies are
utilised to facilitate and empower healthy lifestyles, specifically aiming to increase people's physical
activity levels and decrease inactivity, improve healthy eating behaviour and manage excess weight.

Our integrated service will:
provide a triage function for assessment to direct patients to the appropriate level of intervention
(with a central ‘One Number’)
develop, co-ordinate and deliver a personalised package of care
work collaboratively with other professionals to establish pathways and protocols for referral
between services
pro-actively engage key partners and stakeholders to maximise opportunities to promote healthy
lifestyles for individuals, families and communities
Our integrated weight management service model includes multicomponent interventions, addressing
diet and physical activity together, supported by behavioural change strategies and tools to improve and
sustain healthy eating and physical activity. As an integrated service this model includes:
provision of a universal (Tier 1) service to help support everyone, (regardless of their weight), from
becoming overweight or obese raising awareness of the importance of maintaining a healthy weight,
by eating more healthily and be more physically active
provision of front-line training to ensure consistent messaging and ability to raise the issue
a NICE compliant Tier 2 service providing targeted prevention and early intervention services to those
at high risk of obesity that encourage healthier lifestyles by increasing physical activity and reducing
sedentary behaviour, and improving dietary choices
provision of a specialist Tier 3 service, for adults, children and young people including post bariatric
surgery support, as an enhanced model of the Tier 2 service, including Orlistat prescription
a weight management support service before, during and after pregnancy for women with a BMI
more than 30, working closely with key health professionals, including midwives and health visitors
Service Delivery Schedule, Programming, Locations:
Over the coming weeks, we will be confirming the delivery schedule together with locations of this
integrated service together with specific details regarding referral mechanisms, accessibility and service
documentation.
The launch event of this innovative and integrated service will take place at County Hall on Tuesday 3 rd
