Tuition, mandatory fees and books are expenses that MOWR funding covers. Students may incur
expenses for course related fees and supplies required for a particular course. If a student chooses to
enroll in courses that are not MOWR approved courses, the student may be charged for those nonMOWR courses.
My signature on this form indicates my approval for my student to participate in the MOWR program. I
understand and agree to the following concepts regarding students participating in the MOWR program.



MOWR students must complete the online MOWR application located on
www.gacollege411.org every semester. Failure to do so in a timely manner could result in the
student being dropped from his/her college classes.
Summer semesters cannot be attended by rising freshmen or graduated seniors.
Failure to enroll in and/or pass designated postsecondary courses may result in the student
being dropped from his/her college classes.
Dropping a course while in MOWR may result in the student returning to the high school in the
equivalent class for the remainder of the semester. A decision would then be made about a
student returning to the MOWR program for the next semester by administration.
If a student fails to pass the courses required for high school graduation, he/she will not be
permitted to participate in the May graduation ceremony.
MOWR program courses will become part of the student’s high school and college permanent
transcripts.
The college will not communicate with the parent or high school if a problem arises. The college
will only communicate with the student.
College credits earned while in MOWR are transferable, based on postsecondary agreements
between institutions. Students should be advised to consider the postsecondary institutional
agreements when planning dual enrollment coursework. More information can be found at
www.gatracs.org
MOWR students must continue to meet high school eligibility requirements to participate in
competitive activities.
MOWR students are required to take all required standardized tests and the student is
responsible for making the testing arrangements with the school.

A fourth year science course taken at the college level must be a different course than ones
taken at high school.
Students must comply with all rules from the post-secondary institution and the high school.
Communication of post-secondary grades to the high school, by given deadline is the student’s
responsibility.
The student has the responsibility of contacting both the high school counselor and the college
advisor immediately if the student is considering dropping, withdrawing, or changing any dual
courses.
The parent/guardian acknowledges that the U.S. Department of Education requires that all postsecondary institutions provide training on sexual assault awareness and prevention under Title
IX, 4 C.F.R. 106.

I understand the following information about these required exams:
 I must take the GA Milestones Assessment at my high school in order to receive high school
credit for the course.
 If I do not take the test or do not provide the college grade to my counselor, I will receive a
grade of “I” (incomplete) and will not receive credit for the course. This will prevent me from
graduating.
 The Georgia Milestones Assessment covers high school curriculum. College curriculum will vary
from high school curriculum.
 The Georgia Milestones Assessment will be 20% of the college grade recorded on the high
school transcript. The Georgia Milestones Assessment will not impact the grade on the college
transcript.
 I agree to communicate with the high school counselor or testing coordinator to find out when,
where, and what time the exams will be given and to report on time for the exam.
