Yes Name of the buying organisation: NHS Greenwich Clinical Commissioning Group Description of the contract Description of the services Greenwich Clinical Commissioning Group (GCCG) are looking for innovative ways of delivering key community services for children and young people (birth-18 years) including elements of specialist care in response to children and young people with complex needs and long term conditions. The service will provide coordinated care that contributes to a multi-disciplinary team (MDT) around the child approach, working in collaboration with key stakeholders in primary care , acute services, mental health services, families and individuals to manage their health needs and develop high quality, seamless integrated care 24/7 365 days per year. The service will encompass the following (but not limited to): • Attention Deficit Hyperactivity Disorder Service. • Community Audiology (excluding tertiary audiology) • Community Paediatrics • Community Nursing including Continuing Care and End of Life pathway; • Dietetics • Looked After Children • Music Therapy • Paediatric Occupational Therapy • Paediatric Physiotherapy • Paediatric Speech and Language Therapy • Residential Respite/Short Breaks/Step Down and Outreach Services for Under 5's • Special Schools Nursing (Specialist Schools) • School Nursing (Mainstream Schools) – (Public Health/Royal Borough of Greenwich) • **Additional services may be added to those stated above at a later stage in the process following service reviews with other commissioning organisations **GCCG works in collaboration with other commissioning organisations such as The Royal Borough of Greenwich and NHS England. There are some services that are currently under review which are not included in the above that may be subject to change. The services referred to above will be provided as one seamless service with a single point of access. We would welcome innovative service models including collaborative provision of services, this would require one prime contractor Greenwich Clinical Commissioning Group is committed to meeting key national legislation and their responsibilities set out in the Children and Families Act 2014 and the Health and Social Care Act 2012. A strong joint commissioning partnership and children's commissioning framework is in place with the Royal Borough of Greenwich in order to ensure joint working across agencies, service and institutions. The service will be required to proactively work with GCCG and the Royal Borough of Greenwich to fully develop strong integrated services across the borough that clearly demonstrate and meet the core principles of the above guidance. The service will also be responsible for meeting their specific duties outlined in the above legislation and will establish a robust process within the service to promote, identify and review opportunities for integration and work closely with GCCG to implement key service changes to better meet the needs of children, young people and families in Greenwich The service will demonstrate and meet the key principles set out in the Greenwich Children and Young People's Plan providing high quality health outcomes: RESILIENCE We want all children and young people to engage successfully in education, develop skills for employment, and choose healthy lifestyles. These strong protective factors will ensure children and families to have resilience to cope with any problems they encounter. PREVENTION: We know we will have more chance of helping children and families if we identify their problems swiftly and intervene early to support them to regain their resilience. We want our services to be skilled in identifying families' problems as they emerge, and break the cycles that make them become entrenched. PROTECTION: For some children and families we cannot prevent problems escalating and presenting much greater risks to children's well-being. Taking swift, decisive action will be important to prevent significant and lasting damage to these children's welfare and life chances. It will also maximise our chances of restoring their resilience, enabling them to lead successful lives. The key service outcomes we are seeking (but are not limited to) are for providers to: • Improve the resilience, health and well-being of children and young people including those with specialist or complex needs and/or long term conditions. • Focus on prevention and early intervention by offering the right care at the right time and making every contact count for children and young people and their families. • Drive and implement the multi-disciplinary team around the child approach for every child throughout the whole service, to best meet their health needs and improve health outcomes. • Engage, consult and reflect children and young people and families experiences in the design and on-going delivery of services putting designing the service around the needs of the patient. • Actively collaborate, drive and implement the integration agenda with key stakeholders ensuring children and young people receive the right care at the right time making every contact count by developing a seamless 24/7, 356 days per year service. • Effective, timely, inclusive transition planning for children and young people transitioning to adult services. • Safely manage the health and well-being of all children and young people, protecting vulnerable children and young people and safeguarding against harm. • Reduce hospital admissions and lengths of stay by preventing the need for admissions and focussing on providing care closer to home or within the home. The service will not provide the following; • Assessments for children/young people with no nursing needs i.e. they require pure social care intervention. • Those requiring CAMHs other than services for ADHD and eating disorders which do not require residential admission; and • Assessments for adult services. Transition planning should be undertaken with adult services leading the assessments process. • Registration of Interest To receive more information on this procurement interested parties must register their interest to the CCG Procurement e mail address (GRECCG.procurement@nhs.net).
