Sample Chapter Charter
The following is an example of a charter you may want to use when setting up your
Chapter. You should customize the charter to reflect the unique aspects of your Chapter
and to meet your school’s requirements.

Article I: Name
Section I
The name of this organization shall be _________ University Chapter of IHI
Open School of Health Professions.
Article II: Purpose
Section I
The purpose of the IHI Open School of Health Professions is to engage health
professions students in learning about the principles, methods, and tools of
patient safety and quality improvement.
The purpose of this Chapter is to advance the understanding and commitment of
the next generation of health professionals to patient safety and quality
improvement in health care locally, nationally, and globally.
Article III: Membership
Section I
This Chapter will allow membership to all health professions students committed
to improving the quality of health care. Chapter members are encouraged to
attend Chapter meetings and participate in all IHI and Chapter sponsored events.
Article IV: Meetings
Section I
Meetings will be held on a (weekly/ bi-weekly/ monthly) basis throughout the
academic year.
Section II
Decisions at all meetings regarding any action or future actions of the group, or
regarding the philosophy of the group, shall be reached by consensus of all
present members.
Article V: Officers and Elections
Section I
The officers of the organization shall be:
Chapter Leader (president):
If necessary: (Vice president/ secretary/ treasurer/ chair/ co-chair/ committee
chairs)
Section II
The Chapter Leader shall facilitate all meetings and shall have the authority to
act on behalf of the organization when matters require immediate action.
Section III
Elections will be held during the (insert date i.e. last month of each semester). In
the event of a tie (insert procedure on what will be done at a tie i.e. officers will
have final vote, discussion will ensure, etc.)
Section IV
In the case that any member feels that an officer is failing to adequately fulfill
his/her duties (insert procedure on what will be done to replace an officer if need
be)
Section V
If the Chapter Leader becomes vacant during the semester, (it will be their
responsibility to assign a new lead to step in / elections will be held amongst
Chapter members).
Section VI
A (simple majority/ two thirds majority) vote of all regular voting members will be
needed to be elected.
