Application Instructions for Admission to the Undergraduate Major in Speech-Language Pathology

The following instructions are provided to assist you in turning in a complete application packet. An incomplete
application will be returned to you and delay the admission process.
Use this form if you have an overall coursework GPA of 3.0 or better and have completed the following courses
with a cumulative GPA of 3.0 or better and a grade of “C” or better:
CD 1000 Introduction to Communication Disorders
CD 1800 Observation of Clinical Practicum
CD 2301 American Phonetics
CD 3301 Anatomy and Neurology of Speech-Language
6 hours of English Composition (Engl 1020 & 1030 or Engl 1080)

Personal Information:
Complete all information. University address and phone is your address while you’re in school (i.e. Residence
hall, Warrensburg apartment, cell etc.) If University Address and Phone are the same as Home Address and
Phone, you may put “same” in University Address and Phone spaces.
II. Faculty Appraisals
The Faculty Appraisal form is located on the Communication Disorders Bb Site. You must submit two,
completed by instructors in the Department of Communication Disorders who are familiar with your
coursework.
III. Four-Year Plan
A typed semester by semester plan for taking all necessary courses must be turned in with your application.
You should meet with the Departmental UG Advisor with any questions you have about your undergraduate
program. Provide your signature at the bottom of the Four-Year Plan to indicate that you plan to follow the plan
as you have presented it. The Department UG Advisor will check the plan for concerns related to CD courses.
IV. Academic Information
All information on this section except the speech, language & hearing screening date will be available on the
degree audit.
GPA is cumulative GPA.
Include a current copy of your Degree Audit with your application. You may obtain a current copy of your
degree audit (DARS) online at UCM.edu.
V. Admission Course Requirements Accepted for Transfer – this section is to be filled out only if you are
transferring English composition courses
For the UCM English course(s) listed for which you have approved transfer credit indicate:
Under “Course #” – put the prefix & # of the course you are transferring
Under “Course Title” – put the title of the course at the university from which you are transferring
Under “S.H.” – put the number of semester credit hours the course was worth at the university from which you
are transferring
Under “Grade” – put the grade received for the transfer course
Under “Where Taken” – put the university from which you are transferring the course
Example: Assume you made an “A” in a 3 credit hour course called Introduction to Writing from MIDUSA
University and that the course # was ENG 101. You would fill out the English Course Substitution part of
Section III as follows:

VI. Communication Disorders courses approved for transfer to UCM: obtain applicable instructor signature for
competency approval
This section must be filled out if you are transferring Communication Disorders courses.
It is filled out the same way as Section III with the exception of the UCM CD Course Equivalent & Instructor
Signature/Date. You must fill in the UCM CD Course Equivalent (see the catalog for the appropriate prefix &
number). You must meet with the UCM instructor responsible for the UCM equivalent course. That instructor
will sign off on the course once you have completed any competency performance measures applicable to that
course.
Example: Assume you made a “B” in a 3 credit hour course called Introduction to Audiology from MIDUSA
University and that the course # was SP 301. You would fill out the Communication Disorders Substitution part
of Section IV as follows:

Once you have completed this form, attach a current copy of your degree audit and turn it in to the mailbox of the
Undergraduate Admission Committee, Chair (Blaine Duesing) at the Front Desk of the Center for Communication
Disorders and distribute the faculty appraisal form to any CD faculty member from whom you have taken a course.
The faculty member will return the appraisal to the admission committee.
