Mission Statement:
West Coast Phlebotomy Inc is a Private Career School that is committed to providing high quality professional
training and certification in healthcare education. By our continuous High Standards and insistence on Integrity and

Phlebotomy as a Career:
A phlebotomist is a healthcare professional that collects blood specimens for the purposes of laboratory testing.
Additional duties may be performed by a phlebotomist depending on the clinical setting they work, such as clerical
duties, point-of-care testing, collection and transport of other types of laboratory specimens, and specimen
processing. Phlebotomists may work in hospitals, physician’s clinics, blood donation centers or laboratory collection
sites.
Professionalism and effective communication skills are an essential part of working as a phlebotomist.

With over 20 years of experience in various settings of the health care field, Debbie
considers herself to be a positive person who adapts well to any situation. Being detail

constant state of learning which has enabled her to enhance the quality of the classroom
environment. As a business owner for many years she knows the value of excellent
customer service. Her roots are here in the Pacific Northwest, having been born and raised
in the Portland area. She has a strong dedication and respect for the health care profession.

City's South Ridge Center at 1678

a 2100 sq ft. Professional suite with access
to I-205 and I-5. Many restaurants are within walking distance.
Students will be
provided with state of the art equipment
and updated materials. Students will also
enjoy a spacious classroom, access to a
full laboratory with one on one
instruction, a capillary lab, kitchen and
break room.
Students will have access to the classroom computers, phlebotomy related web sites and to class instructional
materials such as books, microscopes, and practice materials. This list is not inclusive as new materials are added
on a regular base. These materials are for use in the school only.

Students need to purchase their own white knee length lab coat and stethoscope. The estimated additional cost for
these items will be $60.00 in addition to the $2635.00. Students may purchase these items from the school.
We also offer a Health Care Provider CPR class at the school for the cost of $60.00.

Student scholarships: may be available through funding agencies such as Work Source, Vocational Rehab and
WIA. You may contact these agencies through your local Employment Department-Portland Community College or
Clackamas Community College. We currently do not offer any financial assistance or payment plans.
Banks and other lenders may make a loan available to you. Look for scholarships from businesses, or
community organizations, Workforce Development, Vocational Rehabilitation, or similar
agencies in your county. Industries that are closing or putting employees on lay-off may
also pay for retraining in another line of work.
We do not have a federal “school code.” Only very large schools qualify for the
government student loan program. Our small size does not qualify under the government
student loan program. The upside to our small size is the personal attention you get in
class!
Course Description:
This course is designed to provide basic knowledge and skills necessary to perform venipuncture (phlebotomy) in a
clinical setting. In addition, this course will prepare students to obtain phlebotomy certification from the National
Health Career Association (NHA). Students receive instruction in patient care and professionalism, basic anatomy
and physiology, medical law and ethical issues, safety and infection control, HIV/AIDS, HIPAA, blood collection
equipment, and venipuncture procedures. Venipuncture procedures are performed in accordance with current
standards under the guidance of instructors using fellow students. All students must be willing participants as
providers and receivers of phlebotomy.
Classes are held Mon-Thu from 8am until 1:30pm for Aprox 6weeks and a minimum total of 110 class/lab hours.
Course Objectives:
Upon successful completion of this course, students will be able to:
1. Identify the role and function of a phlebotomist in the health care team. (Patient care and Professionalism)
2. Demonstrate knowledge of safety and infection control.
3. Understand and follow OSHA Universal Precautions and Standard Precautions.
4. Demonstrate basic understanding of anatomy and physiology as they relate to general pathologic conditions.
5. Demonstrate understanding of the importance of patient identification in specimen collection.
6. Determine appropriate venipuncture and capillary puncture sites.
7. Identify and select appropriate collection equipment and additive collection tubes used in venipuncture.
8. Perform venipuncture using the evacuated tube method, winged infusion and syringe method.
9. Identify and select appropriate capillary puncture equipment.
10. Perform adult/infant capillary punctures.
11. Urinalysis and centrifuging
12. Blood Pressures
13. Understand and follow HIPAA (privacy laws)

Certificates:
Upon successful completion of this program the student will receive a certificate of completion from West Coast
Phlebotomy. The student will also receive certificates in the following areas: Customer Service, Blood Borne
Pathogens, HIV/AIDS and HIPPA. This program is also approved by the NHA and students may choose to take the
NHA certification exam upon successfully completing this course. Most hospitals will require a National
Certification so it is highly recommended that the student take our study class and then the National Certification
Test both are presented at the school. The cost of the study class is $140.00 and is a six hour class designed to help
the student focus on the material they will be tested on. Study books and materials and continuing education
credits are included as well as meals.
Students will be given information regarding registering for the NHA certification exam during class. The cost of the
exam is $105.00 and will be given at the school via paper and pencil.
Admission & Enrollment Process:
A pre-enrollment interview is required to be considered for possible enrollment. This interview is intended to
introduce the potential student to our program and to make sure the student has the potential to be successful
in this career. ** (See list below)
We keep our classes small to ensure one on one training in our laboratory. Typically 12 to 16 students per term.
Applicants must be at least 18 years of age and have graduated high school or hold a Graduate Equivalency
Diploma (GED) by the time of enrollment. Applicants must be citizens or legal residents of the United States or its
territories. Picture ID is required. Students will be required to provide documentation that both of these
requirements have been met. A TB test result within the last 6 months of the term the student is applying for,
tetanus within the last ten years and proof of the Hep B series are required and are listed on the application. The
applicant is not required to show proof of immunizations before the interview. A CPR card for Healthcare providers
is required before graduation and will be offered at the school for $60.00.
Applicants who are denied or accepted into the program will be notified by email or phone within 3 business days
of the interview. Accepted students will be given a time line in which to complete the requirements listed above. If
applicants are denied admission to the program for any reason they may reapply for a subsequent term. Admission
to the program is by granted by the Director. West Coast Phlebotomy does not discriminate on the basis of age,

Application for each term is open and there is no application deadline prior to each program start date, although
applicants are encouraged to apply at least three week prior to the term they would like to interview for.

When the school receives the completed application along with a $25.00 application fee we will call the applicant

Transfer of credits:
West Coast Phlebotomy does not offer credit for previous training from other facilities.
Transcripts /Academic Records:
Academic records are available to the student/graduate upon request. One official and one unofficial
transcript are provided free of charge upon graduation. Additional requests for transcripts must be in
writing, with a signature, and accompanied by a $10 fee per copy. Student records are released by WCPI
only with written consent by said student and under applicable conditions of the Family Educational
Rights and Privacy Act (FERPA).
**Additionally, prospective students should ensure that they possess all of the essential functions in the
following list to provide reasonable assurance that they will able to complete their course of study and
participate fully in the required training:

3. Sufficient motor coordination to manipulate equipment and safely collect and process patient specimens.

Safety Notice and Statement of Physical Risk:
The clinical portion of this course introduces students to the proper and safe use of venipuncture equipment,
including needles and lancets. There is an inherent safety risk when using these devices and as such, students will
be held to strict guidelines about sharps handling and disposal. Disregard for safe practices as described in the
Student Code of Conduct section will result in disciplinary action. Typically a verbal warning is issued, followed by a
written warning and corrective action plan is introduced. Depending on the situation the student may be expelled
from the course with pro-rated tuition refunded according to the tuition schedule.
This course incorporates the practice of invasive procedures. As stated previously, all students must be willing to
participate both as providers and receivers of phlebotomy. The instructor reserves the right to exclude any student
from performing or receiving a phlebotomy procedure based upon the appropriateness of the situation. Doing this
ensures that all students are treated fairly and safely.
Physical risks of participating in the class include discomfort from venipuncture and capillary puncture possible as
well as exposure to potentially infectious materials.
Students with excessive absenteeism or inability to adequately achieve the required objectives may necessitate
withdrawal from the course. Students may be exposed to potentially infectious materials and therefore Standard
Precautions and Universal Precautions must be followed at all times to ensure safety of all students. Strict
observance of these precautions is mandatory. Failure to comply may result in expulsion from this course. If
exposure does occur, students and staff are expected to follow the procedures for exposure as described in the
safety manual.
Standard Precautions and Universal Precautions:
The CDC recommends the use of Standard precautions to prevent transmission of microorganisms from known and
unknown sources in the healthcare setting. Blood and most body fluids and secretions are considered potentially
infectious regardless of the known status of the source patient. For the purposes of this course, Standard
Precautions will be applied when performing all phlebotomy procedures and will include the use of hand washing,
hand sanitization and appropriate personal protective equipment such as gloves, gowns and masks.
OSHA Blood borne Pathogens Standard uses Universal Precautions as an approach to minimize the risk of
transmission of the blood borne pathogens HIV/AIDS, Hepatitis B and Hepatitis C. Universal Precautions requires
that all blood and body fluids be treated as potentially infectious. For the purposes of this course, Universal
Precautions will be applied using , hand sanitization, hand washing when hands are visibly soiled with blood,
personal protective equipment such as gloves, gowns and masks, work practice controls such as surface
disinfection and engineering controls such as safety devices on equipment and proper disposal of regulated bio
hazardous waste in sharps containers.

Student Code of Conduct:
West Coast Phlebotomy Inc is committed to creating a safe environment that promotes learning as well as
teaching. It is necessary that each student behaves in a safe, ethical, respectful and professional manner. Unsafe or
unprofessional behavior will result in disciplinary action. Initially the student will receive a verbal warning either

from the instructor, or program director. Further incidents will result in a written warning and the instructor or
program director will meet individually with the student to create a corrective action plan. If the problem
continues, the student’s enrollment in the course may be terminated and pro-rated tuition refunded according to
the cancellation and refund policy. West Coast Phlebotomy Inc, reserves the right to implement reasonable
corrective action which may deviate from the general description above as necessary depending upon the severity
of the situation.
Examples of unsafe or unprofessional behaviors include, but are not limited to:
1. Excessive tardiness/absenteeism.
2. Inappropriate attire, see Dress Code Policy.
3. Failure to follow written and verbal instructions
4. Threatening, aggressive or hostile behavior directed at fellow students or the instructor.
5. Behavior that interferes or disrupts teaching and learning.
6. Academic dishonesty such as cheating, fabrication, tampering or plagiarism
7. Attending class under the influence of any drug or substance that impairs judgment or may interfere with safe
performance of venipuncture procedures. Attending class under the influence of any alcohol or illegal drug.
8. Performing venipuncture procedures without appropriate supervision.
9. Disregard for Universal Precautions, Standard Precautions or proper handling of sharps.
Additionally, students are expected to receive feedback from instructors in a professional manner. If a student has
concerns over the feedback received in class, the student is to address the problem according to the process for
student complaints.
Dress Code Policy:
Students are expected to maintain a well-groomed, neat appearance. All clothing must be neat and clean and
students are expected to refrain from wearing perfume or cologne to class. Fingernails must be kept short and
clean as we adhere to strict infection control policies.
Required attire includes healthcare uniforms (scrubs) free of company logos. Close-toed shoes are required due to
the potential for exposure to bio hazardous material. Hair (including facial hair) should be neat and clean with long
hair tied back during laboratory hours.

Cancellation and Refund Policies: (OAR 581-045-0036)
A student may cancel enrollment by giving written notice to the school. Unless the school has discontinued the
program of instruction, the student is financially obligated to the school according to the following:

a. If cancellation occurs within five business days of the date of enrollment, and before the commencement of
classes, all monies specific to the enrollment agreement shall be refunded;
b. If cancellation occurs after five business days of the date of enrollment, and before the commencement of
classes, the school may retain only the published registration fee. Such fee shall not exceed 15 percent of the
tuition cost, or $150, whichever is less;
c. If withdrawal or termination occurs after the commencement of classes and before completion of 50 percent of
the contracted instruction program, the student shall be charged according to the published class schedule. The
student shall be entitled to a pro rata refund of the tuition when the amount paid exceeds the charges owed to the
school. In addition to the pro rated tuition, the school may retain the registration fee, book and supply fees, and
other legitimate charges owed by the student;
i.e. Pro rata refund means a refund of tuition paid for that portion of the program not offered to the student prior
to withdrawal. The date for determining that portion shall be the published course schedule and the last recorded
date of attendance by the student
d. If withdrawal or termination occurs after the completion of 50 percent or more of the program, the student shall
be obligated for the tuition charged for the entire program and shall not be entitled to any refund;
e. The enrollment agreement shall be signed by both the student and the authorized school official. For
cancellation of the enrollment agreement referenced in (a) & (b), the “date of enrollment” will be the date that the
enrollment agreement is signed by both the student and the school official, whichever is later.
Re-Enrollment:
A student who withdraws from the course or does not achieve the minimum requirements may Re apply to reenroll in the program. If enrollment is terminated by the school for disciplinary issues then re enrollment will not be
considered.
Students wishing to re-enroll are responsible for payment of tuition and fees regardless of prior enrollment. In
addition to submitting a current enrollment agreement and supporting documents, students must include a written
letter to the program director that includes:
1. A brief description of what the student’s motivation is for pursuing phlebotomy training.
2. A brief explanation for withdrawal, or inability to meet minimum course requirements in the previously enrolled
course.
3. An explanation/description of how the student has addressed, corrected or resolved the issue as described
above.
The program director will review the letter requesting re-enrollment and will approve or follow-up with the student
if further remediation of the situation is deemed necessary prior to course enrollment. Re enrollment is not
guaranteed and will be at the sole discretion of the Director.

Attendance Policy
To ensure students are able to successfully meet the course objectives, attendance is required. In the event of
illness or emergency, students must contact West Coast Phlebotomy Inc, immediately regarding the absence. You
must leave a message on our answering service if we cannot be reached in person. There are no make-ups for
lecture materials/quizzes or lab hours. Students should not be absent from class for more than 2 days. After the 2 nd
absence from class, students will be notified that they have been placed on probationary status. Further absence
will result in termination of student enrollment in the course. Students with unexpected physical limitations may be
able to continue with this course; however the student understands that excessive absenteeism or inability to
adequately achieve the required objectives may necessitate withdrawal from the course. It is the students’
responsibility to meet with the instructors and program director to determine appropriate action if
emergency/illness may prevent them from meeting the attendance requirements.
Students are expected to arrive to class on time. Tardiness in excess of 15 minutes may be counted as an absence.
Students are expected to stay for the duration of scheduled classroom hours. Failure to stay for the duration of the
class will be counted as absence.
Grading
Course objectives will be met using a combination of lecture and practical laboratory experience. Students will be
given assignments based on lecture material to complete during class hours as well as outside of the classroom.
Weekly quizzes and tests will provide assessment of student progress. (Incomplete assignments will not be
accepted and will be marked down for being late) The student will also be evaluated for overall performance using
certain attitudes and behavior criteria. (This list of criteria will be discussed in detail and given to the student during
the pre enrollment interview.) During the term the student will be notified as to what needs improvement to allow
students to address areas needing improvement. Student code of conduct applies.

Internships: are limited and offered as sites become available. Students are not automatically enrolled or
considered to be eligible for the internship program. Students must meet certain criteria to be considered
for any internship. (School has list of criteria) Students must have complete documentation of
immunization records, TB testing, criminal background check, drug screen and liability insurance coverage
for an internship. (School has information and check lists for these). Students who participate in the
internship must be available on a full-time basis for three 40-hour weeks. There are no evening or
weekend internships. (Internships are on a volunteer basis).There will be an additional fee of $450.00 to
$500.00 for students selected to do an internship.
Students who participate in the internship program must successfully complete the internship to receive a
certificate of completion from the school. Students have the option to complete the phlebotomy program
without the internship portion and upon successful completion at the end of the six weeks will graduate
and receive their certificate of completion. Background checks and drug screening will not be required if
not applying for the internship program.
Students who successfully complete this phlebotomy program are eligible for employment at
independent labs, hospitals, clinics, various other medical facilities and are eligible to sit for a national
certification examination offered at the school. This program is not a guarantee
of an Internship or employment.

Volunteer Internship Requirements/Criteria
Should I meet the requirements/criteria for a volunteer internship,
I understand that I will be put on a list and be placed according to the location and facility that will best fit
my needs. I am also aware that there may be a waiting period for me to start a volunteer internship. This
will be dependent on the availability of sites and locations. I also understand that I will not receive a
certificate from the school until I have successfully completed the internship.

----Final test grade must be 85% or higher.
----Evaluations of Lab Skill/Professionalism must be 90% or higher.
----Final Practical must be 90% or higher.
----Must pass each unit with an 85% or higher.
----Not more than one tardy during the term.
----Not more than one excused absence during the term.
----Student cannot have any written warnings during the term.
----Must interview with Director and be approved by West Coast Phlebotomy and internship site.
----An internship through this school requires an additional fee paid to the school at the time of
acceptance into the internship program. The fee will range between $400 to $550.00 depending on
the site requirements and length of internship.
10. ----Student must have all required immunizations/insurance/ be able to pass background checks
and drug screens before beginning internship.
11. ----Student must complete internship with an acceptable evaluation from the site in order to receive
a certificate of completion in phlebotomy from the school.
----West Coast Phlebotomy has raised the bar in Allied Health Care Training and expects its
students to rise above that bar to show their desire and professionalism during their phlebotomy
training to be considered for an internship. Since completing an internship is not mandatory to
receive a certificate of completion from the school, it is expected that the student who wants
additional training will meet the criteria above and also abide by the syllabus and amendment to
the syllabus.

Health, Character and Technical Standards
WCPI must ensure that patients/clients are not placed in jeopardy by students during learning experiences.
Therefore, students in the
phlebotomy program as well as internships must demonstrate sufficient emotional stability to withstand the
stresses, uncertainties, and
changing circumstances that characterize patient/ client care responsibilities. Furthermore, the student is expected
to have the emotional
stability to exercise sound judgment, accept direction and guidance from a supervisor or faculty member and
establish rapport and
maintain sensitive interpersonal relationships with employees, patients/clients and their families.
WCPI believes patient and student safety is of utmost importance. Therefore, if the instructor believes that actions
demonstrated by a
student in class or on internship jeopardize either the student’s own safety or patient safety, that student will be
dismissed from the
class/program.
Applicants must be in good physical and mental health to qualify for positions in the health care field. Standards
have been established to
provide guidance to students as to skills and abilities required to function successfully in the program and
ultimately in the phlebotomy profession.

Phlebotomy students must:
1. Demonstrate reading, writing, and listening comprehension competencies in the English language as well as the
ability to speak English
clearly and correctly.
2. Have sufficient hearing to assess patient needs and to understand instructions and identify emergency signals.
3. Have sufficient visual acuity to observe patients, and interpret data; visual acuity sufficient to ensure a safe
environment, identify color
changes, read fine print/writing and calculate fine calibrations.
4. Have sufficient speech and language ability to express, comprehend, and exchange information and ideas in
English verbally, nonverbally, and in writing, and to interact clearly and logically with patients, family members, physicians, peers and
other ancillary medical
personnel.
5. Ability to work with frequent interruptions, to respond appropriately in emergencies or unexpected situations
and to cope with variations
in workload and stress levels.
6. Ability to stand for extended periods of time.
7. Sufficient emotional intelligence to exhibit empathy and compassion, to maintain productive relationships in the
classroom and clinical
settings, and to integrate direction, instruction, and constructive criticism into behavior.
8. Criminal background checks are required of all students prior to entering the clinical internship setting. Drug
screening is also required.
No students will be exempt from either process.
9. Current immunizations.
Students must achieve the minimum grades as well as successful completion of 25 venipunctures and 10 capillary
punctures to pass this course. This is the minimum requirement to receive a certificate and to be eligible to register
for the NHA certification exam. If a student is concerned about their progress in the class, it is the responsibility of
the student to meet with the instructor regarding the concern by making an appointment during office hours.

Inclement Weather/Emergency Procedure
West Coast Phlebotomy Inc may need to enact a school closure due to poor weather conditions or other type of
emergency. To simplify the school closure process, West Coast Phlebotomy Inc, will observe school closure as
announced by the local Community College or University. WCPI school closure will be presumed that the closure
applies only for that 24 hour period and classes will resume as regularly scheduled the following day. In the event
that questionable weather conditions or emergency develops in the afternoon or evening prior to class and it is
determined that school will close, an attempt to contact students regarding closure will be made.
Observed Holidays: WCPI is closed on the following dates: July 4th, Labor Day, Thanksgiving and the following
day, January 1st and Memorial Day weekend.
Student Complaint Process
The Student Complaint Process ensures fair and reasonable resolution of student complaints. Student complaints
addressed by this process may include, but are not limited to classroom instruction issues, administrative services
as well as issues pertaining to discrimination. The student will not be retaliated against as a result of filing a
complaint. If the complaint alleges a violation of law or private career school standards, or a student wishes to
appeal the school’s decision, they may contact the Superintendent of Public Instruction, Higher Education
Coordinating Commission, 775 Court St. NE, Salem, Oregon 97301. 503-947-5751.
1. If applicable, the student should first attempt to resolve the complaint with the person involved in a
meeting outside of classroom hours. Documentation of meeting details is necessary should further action
need to be taken. This step may be skipped in certain situations.
2. Should the initial meeting fail to resolve the complaint, students may request a meeting with the within 10
business days of the incident.
3. Students may appeal to the Program Director any decision made by the director within 10 business days of
meeting with the Director. Appeals are made by submitting a formal written complaint to the Program Director.
The Program Director will contact the student and schedule a meeting with the Student. Decisions made by the
Program Director are final.
4. As stated above, if the complaint alleges a violation of an Oregon law or private career school standards, or a
student wishes to appeal the school’s decision, they may contact the Superintendent of Public Instruction, Higher

Student Grievance Policy: Students aggrieved by the actions of the school should attempt to resolve these issues with the
appropriate school officials. If this procedure fails, students may contact the Higher Education Coordinating
Commission, 775 Court St. NE, Salem, Oregon 97301. 503-947-5751 .After consultation with appropriate school
department staff and if the complaint alleges a violation of Oregon Revised Statutes 345.010 to 345.470 or standards of the
Oregon Administrative Rules 581-045-0001 through 581-045-0210, the Department will begin the complaint investigation
process as defined in OAR 581-045-0023 Appeals and complaints.
Statement of Non-Research Use of Blood Specimens
To ensure adequate privacy of students, West Coast Phlebotomy Inc, does not utilize any human blood specimens
collected as a result of training in this course for research purposes. All human blood and body fluid specimens
collected during this course are disposed of in bio hazardous regulated waste and destroyed by the regional wastehandling company.
