2014-2015 SAFE in H5 Membership Application

Please print all of them AND this cover letter out, complete them, and return them to the SAFE
address above by September 1, 2014 or email to: lk.graham58@gmail.com.
If you are a renewing member, please be reminded that your 180-day card for this past school
year is due by July 15, 2014. If we have not received your 180-day card, we will not accept your
2014-2015 SAFE application.
Membership in HSLDA (Home School Legal Defense) or an organization similar is required for
membership into SAFE. If you are newly applying to SAFE in HERALD 5 and do not yet have an
HSLDA membership, you will need to go to their web site at: http://www.hslda.org to apply for
membership. The application fee will be $100.00 rather than $120.00 if you give them the SAFE
Group Number #299586. Please do not share this number with anyone else as it only applies to
members of SAFE. If you are a returning SAFE member or already have an HSLDA membership,
but need to renew your card throughout the year, you may also use the SAFE Group Number to
do so.
Returning SAFE members: Be sure that your HSLDA membership is up to date. If it is going to
expire in the next few weeks, please re-apply now. If you have already re-applied, but have not
yet received your new card, copy the OLD card now and return it with your application along
with a note saying you have re-applied. Send a copy of the NEW card as SOON as it arrives. If
your HSLDA membership is not up to date, we will be UNABLE to process your new SAFE in H5
application. All SAFE applications MUST have a current HSLDA card attached in order to be
processed.
Please REMEMBER that 90-day-cards, 180-day-cards, active membership in HERALD 5 and
keeping up with your HSLDA membership (i.e. not letting it expire any time during this school
year) is YOUR RESPONSIBILITY. If at any time, SAFE must send you a letter regarding delinquent
paperwork, you will have 10 DAYS to submit the required item. If the item is not returned
within the 10 days, your SAFE membership will be revoked.
The 90 and 180-Day Cards will be sent via e-mail approximately two weeks before their required
return date. The 90-day-card will need to be returned by Feb. 16, 2015; the 180-day-card by
July 15, 2015. Mark these dates as well as the expiration of your HSLDA card on your calendar
now and don’t forget to send all required material on time.
All high school families must submit by July 15th each year, all courses/credits issued, grades
and GPA for each high school grade, 9-12th. A worksheet will be on our website for your use. If
your student desires the Palmetto Fellows Scholarship, please have a copy of their SAT scores
sent to the Commission on Higher Education, code 4313.
SAFE offers a “Needs Based” scholarship for members with special needs children or single
parents. If you are interested in that, please contact me.

**Any parent whose child is under the age of six years prior to 9/1/14 may elect for their child to waive
attendance in kindergarten. The parent must sign a waiver with their school district or their supervisory
organization (such as SAFE).
HERALD 5 SUPPORT GROUP STATUS: Membership in SAFE is contingent upon
active HERALD 5 Support Group Status. Active membership requirements are outlined
on your HERALD 5 application. Membership in SAFE will be denied those who do not
meet those requirements. Virtual Charter Schools are not eligible for membership in
SAFE.
Divorced/Separated SAFE Spouses:
HSLDA has advised SAFE to obtain the spouse’s written approval verifying their
understanding that their children are being homeschooled. This document will be
required before membership in SAFE is accepted.
SAFE in H5 Statement of Commitment
I commit to do the following:

Maintain a current membership with Home School Legal Defense Association (HSLDA).
Be an active member of HERALD 5 Support Group.
Have a high school diploma or GED.
Have an instructional year of at least 180 days.
Plan a curriculum including, but not limited to, the basic instructional areas of reading,
writing, mathematics, science and social studies; and in grades seven through twelve,
composition and literature.
6) Maintain educational records including:
a)
A plan book, diary, or other written record indicating subjects taught and activities in which the
student and parent/teacher engage.
b) A portfolio of samples of the student’s academic work.
c) Semester progress reports including attendance records and grades/evaluations for each
of the subject areas outlined in Point 5 above.
7) Maintain active membership with SAFE in H5 by:
a) Completing enrollment requirements by 9/1/14 (See below)
b) Submitting the 90 and 180-day cards by assigned deadlines.
8) Renew HSLDA membership before expiration of current membership and submit a copy of the new
HSLDA card within 10 days of the previous card’s expiration date.
9) Submit all requested forms/paperwork required by SAFE within 10 days of notification.
10) Make records available upon request.
I understand that failure to fulfill any one of these commitments will result
in termination of my child(ren)’s membership in SAFE in HERALD 5.
