COMMUNITY DEVELOPMENT BLOCK GRANT
APPLICATION PACKET
The City of Logan invites Community Development Block Grant applications. To simplify the
application process, applicants can choose from two application packages, both which have an
applicant information form and one of the following:
PRIORITY 1: NEIGHBORHOOD & HOUSING APPLICATION
Use this application for activities that contribute to neighborhood infrastructure or facility
(including housing) improvements in eligible neighborhoods.

PRIORITY 2: OTHER COMMUNITY NEEDS APPLICATION
Use this application for activities that contribute to social services, nonprofit, or neighborhood
initiatives that address community needs for low/moderate income residents.

Applicants are required to include two years of audited financial
statements (balance sheet and income statement) for the
applicant organization or, alternatively, the applicable unit if the
applicant is part of a larger organization.

Applicants are encouraged to submit a brief overview of your
organizations:
● Mission and programs
● Clientele: number, income, % living in Logan city limits
● Ability, organizationally, to fulfill recordkeeping and other
CDBG requirements
Applicants are encouraged to include:
● An official city department endorsement, such as a letter, if
this project affects city property; and
● Indicators of strong public support, such as petitions and-or
letters.

The application contact will produce information to the CDBG Coordinator to further verify or expound on this CDBG
application in a timely manner.
If a proposed activity does not meet the minimum eligibility requirements (HUD National Objective (one); AND; HUD Activity
Category (one); AND Logan Strategic Objective(s) (one or more) AND Logan Performance Measures (one)), the application will
be deemed “ineligible” and will not be included for consideration for CDBG funding before the CDBG steering committee or
the Logan City Council.
Logan CDBG funding allocations are recommended to the Logan City Council by an appointed CDBG steering committee,
which recommendation is based on majority consensus vote based on a ranking and rating process. The Logan City Council
may uphold or amend the CDBG allocations, and will adopt the final funding by resolution.
Proposed activities are also subject to various, separate consents from the State Historic Preservation Office, US Fish &
Wildlife Service, US Congress, US Department of Housing and Urban Development, and US Environmental Protection Agency.
As such, significant delays which are out of the control of the City of Logan may delay the ability to officially announce funding
and/or to commence the project.
Throughout the entire Logan CDBG allocation process, availability of funding is subject to Congressional approval and award
of funds.
CDBG funding may only be awarded to a city department or eligible nonprofit agency. The awardee will enter into a
subrecipient agreement with the City of Logan and must comply with any HUD certification requirements and other federal
and state regulations mandatory to the operation of the program, as well as all Logan CDBG procedures. Consequently, failure
to submit any documentation requested by the City of Logan to demonstrate program compliance may result in administrative
sanctions up to and including rescission of funding.
Logan CDBG sub-awards are paid on a reimbursement basis.
Public comment or lobbying for or against an application or applicant should be made in writing to the CDBG Coordinator
during the public review/comment period. If a steering committee member is subject to such comments and lobbying efforts,
they are to redirect the persons to the CDBG Coordinator. If the issue is not submitted in writing to and dispersed by the CDBG
Coordinator to the whole steering committee, it is to be ignored by the steering committee to avoid inconsistent
consideration.
There are no outstanding violations related to myself, the applicant organization or its property, with city divisions, including
but not limited to: Building Safety, Neighborhood Improvement, Planning.
Applicant organizations must have a local business license from the City of Logan (435-716-9230), a DUNS number from Dun
and Bradstreet (free at www.dnb.com), and, as applicable, an FEIN and 501(c)(3) from the IRS (www.irs.gov) and be registered
the Utah Division of Corporations.
The applicant organization will permit pictures and other information about this application to be published or otherwise
shared for purposes related to the Logan CDBG program.
Expenditures related to the activity may not be made until clearance is granted from the CDBG Coordinator. Anticipate 1
August 2014 as the earliest possible date, though it is subject to delays as referenced above.
