JOB SUMMARY
Horizon Health Choices Ltd is an independent healthcare provider supporting the NHS in
delivering community-based patient services, and as such, it is committed to providing
high quality care with greater choice.
As a GP Federation that has recently expanded from 26 to 54 member practices, one of
Horizon’s core strategical objectives is to support the collaboration of its member
practices to deliver efficiencies that increase the resilience of General Practice locally.
This role, GP Federation Project Manager, has been created to ensure Horizon Health
Choices meets the changing climate of the NHS and delivers upon this objective.
The post holder will work for the Head of Federating & Business Development with the
aim of optimising income generation from Horizon Health Choices’ assets, and
maximising opportunities for new business development.
KEY RESPONSIBILITIES
Being responsible within their allocated locality/area of work, this role will require the post
holder to be flexible in their approach to ensuring their GP members collaborate by
delivering on the key areas of project management and business development. The
success of GP Federating across the county will include, but will not be limited to, the
following duties and responsibilities:
GP Federating



Both support and lead the development of locality-based and countywide GP
federating plans as a key deliverable.
Devise and implement an appropriate locality strategy and governance framework
to promote ambitious but realistic GP Federation aims and objectives.
Lead and manage locality federating initiatives.

Identify cost efficiencies across the locality and system-wide federation model and
develop plans for implementation in a timely fashion.
Utilise strong change management skills with the implementation of GP
Federations transforming the shape of primary care locally.
Strategy



Under the direction of the Head of Federating, create and then maintain a robust
business planning process for their localities whilst also supporting Horizon Health
Choices in its ambition for excellence.
Once implemented, to be responsible for the day-to-day management and
coordination of the business planning cycle, providing planning and project
management expertise and communicating progress and regress to the Head of
Department regularly.
Evaluate and risk-assess emerging service options in the context of Horizon
Health Choices’ strategy for their locality.
Service Improvement




Interrogate IT data in relation to services deliverables and KPIs to establish a
robust understanding of the services to enable constant service improvement to
achieve cost efficiencies and quality of service provision.
Work closely with the Head of Federating, Head of Operations and Operations
Manager to identify and implement areas of improvement in a timely fashion using
change management techniques and expertise.
Develop business cases, implement and monitor agreed service changes under
the direction of the Head of Federating.
Ensure that public and patient user views are taken into account in the planning
and monitoring of services.
Business Development



To identify new and emerging business opportunities relevant to Horizon Health
Choices’ localities and their core business. As a result, propose priorities for new
business development and work with business development colleagues to build
innovative proposals.
Support the Head of Federating & Business Development with tender submissions
and proposals to generate new business as necessary.
Develop and maintain strong relationships with clients and suppliers, ensuring
relationship management is robust.
Project Management




To project manage locality-based and countywide initiatives and new service
models to be delivered on time and within budget.
Ensure the identification, escalation and resolution of risks and issues.
Actively influence, motivate, develop and support key stakeholders.
Develop and maintain appropriate project management documentation to ensure
this is successful, such as a risk register (to identify, address and alleviate any real

and future risks), a stakeholder matrix (to identify key stakeholders and document
their individual needs and concerns) and robust mobilisation plans.
Information and Communication
In line with any agreed service developments or projects, manage the collection and
analysis of information relating to performance activity and quality, identifying correct
measures where appropriate to meet Horizon Health Choices’ standards of excellence.

Professional Registration/Codes of Conduct: Where your role requires you to maintain a
statutory professional registration, it is your responsibility to ensure that this is maintained. The
continuation of your employment is conditional upon you continuing to be registered with the
appropriate professional body. Horizon Health Choices’ will require evidence of current
registration. Failure to comply may result in your suspension from duty and could result in
disciplinary action.
