Cardiac examinations by Dr. Megan King, VMD, Diplomate of ACVIM, Board Certified Cardiologist
Cardiac auscultation ($50 cash) and Cardiac echocardiogram/ Doppler ($240 cash.) Fees do NOT include OFA registration. Bring
completed OFA Cardiac Certification form, downloaded at www.offa.org
OFA or CERF examinations by Dr. Martha Low, DVM, Diplomate of ACVO, Board Certified Ophthalmologist ($50 cash)
Bring dogs’ registration papers and microchip/tattoo numbers. Report to exam area 15 minutes before appointment to apply drops to
dilate the dog's eyes. Fees do NOT include OFA/CERF registration. See www.offa.org or http://web.vmdb.org/home/ for information.
Laboratory blood tests interpreted by Dr. Jean Dodds, DVM and associates of HEMOPET/HEMOLIFE.

Results of each sample are reviewed and reported by the Hemolife staff on our unique Cloud-Based computer technology
Laboratory Information System (LIS). They are then personally reviewed and interpreted [taking into account age, sex, and
breed-type, and activity of the animal] by Dr. Jean Dodds and two trained veterinary colleagues, Drs. Andrew Zuckerman
and Gary Richter. If further consultations are required for these cases, veterinarians or pet owners can discuss the case with
Dr. Dodds and Staff at no additional charge.

Please schedule your appointment by February 17 and direct all questions to Shana at
WalkerRR@verizon.net or (215) 499-0099. Minimum of 25 dogs for each cardiac and CERF testing, or that portion of the
clinic may be cancelled. Walk-in registration will be accommodated if available for additional $10 per service per dog.
All payments must be in CASH. No credit cards or checks accepted, except those directly to Hemopet and OFA.
