The above named student is requesting approval to invite a guest to Gulf Coast High School’s Prom who is not currently a
Gulf Coast High School student. By signing this form the Gulf Coast High School student agrees to follow all school rules
and regulations and shall be responsible for his/her guest’s behavior. Guests must currently be enrolled in the 9th-12th
grade in a Collier County Public or Private School, or be a graduate of a Collier County School in 2013 or 2014. No guest
may be over the age of 20 years old, even if they meet other requirements. Seniors can bring a guest from out of county
as long as they meet all other requirements. All students must have a GPA of at least a 2.0 and not have any outstanding
fines or overdue books.
All guests must have a valid picture I.D. in order to be admitted at the dance. If your guest does not have a picture I.D.
you must make special arrangements with the Activities Office. A copy of the picture ID must be attached to this form.
All guests must agree to conform to the dress code standards of Gulf Coast High School.
Deadline: This form must be completed and returned to the Activities Office by 7:30 am on Thursday,
April 7, 2016. The appropriate class sponsor will then be notified of your eligibility to purchase tickets.
