Criteria to Include or to Offer Participation in
The Paired Kidney Exchange List for Compatible Donors

Purpose
The 3 Site Kidney Transplant Convergence Group has developed written guidelines
for criteria to be considered for including compatible pairs in the Paired Kidney
Exchange list. All Protocols have been reviewed by the Convergence Group and
approved by the individual sites
Policy Statements
The following principles are to be used when considering when to ask compatible
pairs to participate in Paired Kidney Exchange
1. Principles:
a. Assure recipients’ benefit/non inferior graft
b. Assure timely transplant
2. Criteria to include in PKE:
a. Presence of a positive crossmatch
b. ABO incompatibility
c. EBV mismatched pairs
d. Negative crossmatch with high level DSA
i. Current Class I with MFI >2000
ii. Current Class II with MFI >1000

3. Criteria to offer to participate in PKE:
a. CMV mismatched airs
b. Pairs who are less favorable immunologically
i. Low level or historical DSA
a) Current or historical Class II DSA at any MFI
b) Current Class I DSA with MFI <2000
ii. Positive crossmatch and no identified DSA by Luminex
c. Donor age >10 yrs older than recipient, provided recipient age<50 yrs
d. Size/GFR mismatch
Transplanted graft GFR<50 ml/min/1.73 m²
Transplanted graft GFR= [donor unadjusted iothalamate clearance
(ml/min) /2] * 1.73/recipient BSA
e. Other criteria as determined at selection conference such as
surgical/anatomical suitability
f. Altruistic compatible pairs
