Dear Parents/Guardians,
The safety of our students is extremely important to us at Eisenhower. Understanding
that there are many students with food based allergies as well as religious restrictions,
we want to take a moment to share with you a list of food items that might be for sale
during the lunch periods throughout the school year. Having this list will allow you to
work with your child to ensure that the foods they eat at school are within the guidelines
and health safety parameters that you have established for your child. We have also
included specific brands or restaurants so that you can check ingredients to ensure that
your child will not have an allergic reaction.
Moving forward, starting in December, we will need written consent for each child in
order for them to purchase items that are for sale in the lunch room. This does not
include the PTA hot lunch program; your signature on the hot lunch form indicates your
consent for the food being purchased.
It is our goal to ensure that our students are safe. In order for your child to receive
the treats listed on the reverse, we need your permission. Please sign, date, and
return this form by Monday, November 30, 2015.
