QUEST IN HEALTHCARE
Kolkata: April 13, 2011: The Calcutta Medical Research Institute a leading edge
healthcare provider of eastern India shared the medical feat achieved by their
medical team recently.
Mr. Anand was having a good life till a year back. He was a tea taster and doing well
in life. He was getting a fat salary and had a good lifestyle. He like most young men
believed his health is not going to fail him ever. He couldn’t have been further from
the truth. 10 months back, he had an acute abdominal pain. He felt faint and had to
be hospitalized in CMRI. The treating doctor, Dr Sujay Mukherjee, MD, SNB Critical
Care found him in a state of shock. He was resuscitated and investigations carried
out to detect his disease. Tests in the form of CT angiography revealed clots in the
abdominal and right limb vessels. This had stopped the blood flow to the gut and the
right leg . This was a dangerous situation. In medical terms it was a Thromboembolic
disorder where blood vessels get clogged and the area they supply becomes
gangrenous spreading deadly toxins in the body. Many may not survive this assault.
Medicines to dissolve the clots were started promptly. Clinical evaluation revealed
rapid deterioration in his condition signifying deadly toxins in his blood. Decision was
taken to sacrifice his limb and gut to save his life. He was operated on 10/06/2010.
His right leg was amputated. His abdomen revealed 5 meters of gangrenous gut,
which was excised. This left him with less than 2 meters of healthy gut. The two
ends of gut were not joined then, as the patient was too sick for normal healing. It
was brought out of the abdomen and fixed to the skin from where stool was
collected in a bag. Post operation he recovered well. Since the gut left behind was
too little for survival, he would defecate almost everything he ate. Thismeant to
ensure food reaching his blood, apart from the oral overfeeding, food had to be
given directly into the blood. this was done via a catheter placed at the neck.
He did well on this strategy. The only concern he had were the two bags on his belly.
One of them would fill up as soon as he ate and he was tired emptying it. It made
him immobile. The doctors explained that if the two ends were connected, he would
need to go to the toilet at least 10-20 times a day. Of course they would try to
improve on his plight as much as they can by innovative surgical technique and
medicines.
3 months after the first operation, he underwent a second surgery to join the two
ends. He recovered well. As expected he had 10 -20 motions a day which settled
down over the next 3 months to 6-10 times a day.
A port was also placed subsequently beneath the skin at the chest, which
communicated with the heart directly via a catheter. He uses this everyday to instil
food (glucose, proteins) into his blood.
Ten months hence, he has joined his work now. He eats and drinks every two hours.
He instills food containing bottles every night to nourish him adequately. For a man
without a leg and most of his intestines, Mr. Anand has done well. A salute to his
efforts and resilience is in order.
Dr Sarfaraz Baig, MS, FRCS, FIAGES, Gastrointestinal, Laparoscopic & Bariatric
Surgeon and Dr Ajay Mandal, MS, in CMRI who did this operation said “ Most
patients with this disease die during the hospital stay due to infection. Some who do
survive do not live more than a few weeks or months due to malnutrition and salt
imbalance in the body. This is indeed a rare case of survival.”
Dr Baig added “ This is the second case in our center who has survived. The first
patient, Mr Vijaya Maddala is now living over two and a half years. He is maintaining
his weight and taking night feeds via catheter on alternate days. He has rejoined
work in a nationalized bank in Vishakapatnam.”
Dr Sujay Mukherjee said” We are probably getting better results because of
improved care of nutrition in these patients. A dedicated gastrosurgery team is
obviously of paramount importance in complicated abdominal cases such as these.”
Mr.Rupak Barua, Chief Operating Officer, CMRI opined, “We constantly encourage
our medical and paramedical team members to imbibe the latest happenings in
the field of medical science towards better delivery of medical service.”
