Note From Our Course Director
Dear Colleagues,
It is with great pleasure that we are introducing our new name:
CASCADE™- 2015: Advances In Hemostasis & Thrombosis.
This annual meeting, as we have over the past years, will continue to be
a comprehensive one-day conference. We will concentrate on the
latest science, trends, and recommendations for the prevention,
diagnosis, and treatment of both thrombosis and hemostasis issues.
By annually gathering physicians and health care professionals across
multiple medical disciplines, our intent is to keep the attendees at the
forefront of the latest information on hemostasis and thrombosis.
Our speakers and topics this year should again provide an abundance of
the latest information, allowing for provocative questions and lively
discussion, assisting each of you in the care of your patients.

All registration forms MUST be postmarked by April 24, 2015 to preregister for this conference. We cannot guarantee space in the program
or any program inclusions for those registrations received after this
date.
Payment must accompany your registration.
Cancellation/Refund Information:
All activities accepting tuition payments using the CCEHS registration system must
include refund/cancellation information. Sample text is included below.
All cancellations must be in writing and sent to us via U.S. mail, email or fax (see
contact information below). Tuition for cancellations postmarked or date stamped
before April 17, 2015 will be refunded in full. After April 17, 2015, a $35.00
administrative fee will be assessed and the remaining amount refunded - and no
refunds will be made after April 25, 2015.
