FAST Appeal Form
This from is for those seeking permission to miss a mandatory event hosted by FAST or an event that DSTAFF has named mandatory. Everyone is allotted two unexcused absences for mandatory events,
GCMS, and Sub-Committee meetings and only one of these absences can be used for an event under
emergency situations. Please turn this from in to D-Staff no later then a week before the scheduled event.
You do not have to fill this form out to miss a GCM or Sub-committee meeting but you do have to let DStaff know ASAP if you need to miss a GCM or your sub-com counselors for your sub-com meeting.

I understand and agree that the Director Staff will review the information and make a decision. The
decision made is final. I also understand that the Director Staff can request documentation for verification
