Subj: ADDENDUM REQUEST TO MISS CLASS TIME
1. I respectfully request permission to miss class on Friday, 07 Nov 2014 to travel on
Movement Order 1403372 to Philadelphia for the purpose of YP Squadron training and support
of Navy athletics. Below you will find the signatures of all professors whose classes I intend to
miss for the time period of concern and their approval or disapproval of my absence. I
understand that it is my responsibility to coordinate with my professors to ensure that my
absence minimally affects my academic performance.
2. FOR PROFESSORS: This is a request to be absent from your class on Friday, 07 Nov 2014
to travel on Movement Order 1403372 to Philadelphia for the purpose of YP Squadron training
and support of Navy athletics. I understand that if you approve, I am responsible for any material
covered in class and any work assigned. If you believe that my absence from this class will
create significant academic problems for me or significant administrative problems for you, I
will understand your disapproval.
