SUBJECT: Request Waiver for TIS/TIG for Civil Affairs Assessment and Selection and
Qualification Course for SPC John Doe, 1234.
1. Request exception to policy in regards to TIS/TIG. My date of rank to SPC is 01 December
2009 and my basic active service date is 07 May 2008. This date of entry and time in grade
places me outside of Civil Affairs recruiting guidance. Without this exemption to policy, I will not
be able to attend CA Assessment & Selection to attend the Qualification Course.
2. I am still within the respective time to invest in a productive, dedicated career to the Civil
Affairs Team, Operation & cause.
