Adults (≥18 years of age) must register individually -- Example: neither spouse nor partner may

Dependents must be registered at the same time as the parent/guardian. The parent must
sign the consent form for dependents if under 18 and the relationship should be assigned
(parent/guardian). To register a dependent, click the “Add Child” button located at the bottom
left of the registration form. (Note: as soon as the registration form is signed, a dependent can

•
Registrants will be electronically signing consent for themselves and their dependents.
•
Registrants must note the email and password or username and password configuration that
they create during registration. This will provide the registrant access to the solution if he/she

•
The registrant should use Google Chrome or Mozilla Firefox when using the online registration
tool.
