Checklist for Out of Province SW Student Placement in AHS
Student placement requests should be made 3- 6 months prior to the placement start date. Failure to
comply with these deadlines will limit our ability to locate placements.
 All students participating in a student placement must be covered by a legal contract called a Student
Placement Agreement (SPA). A template of this agreement can be found at:

When requesting a placement, please include the following as part of the request:
 Completed HSPnet Placement Request Form - faculty (If the student is interested in more than one
site/program, please indicate alternates in comments section) and Student Consent Form:

 Student Cover Letter and Resume
If accepted, all students must have the following in place prior to commencing a placement with
AHS:
 Immunizations: All health care students must be immunized against rubella, verified through
immunization records. AHS strongly recommends immunization against:

tetanus, diphtheria, pertussis, (DTP)

measles, mumps, rubella, (MMR)

Varicella (chicken pox)

Influenza

Hepatitis B (HepB), Polio, Tuberculosis (TB); for some disciplines
 Police Information Check including a Vulnerable Sector Search: If a student’s PIC is “non-clear,” the
educational institution must submit an AHS Security Screening Adjudication Intake Form to
StudentPIC@albertahealthservices.ca to request a risk assessment
 Signed Confidentiality Agreement: Confidentiality training includes watching a video and completing
the associtaed online learning module: http://www.albertahealthservices.ca/3962.asp.
 Reviewed AHS policies applicable to students:

 All students have current CPR (if applicable)
