COUNTRY OF ORIGIN LABELING (COOL) AFFIDAVIT:
COOL Law became mandatory September 30, 2008. 4-H & FFA market livestock (beef, sheep, swine, and goat) projects
must provide information to document animal origin to buyers. All projects should also maintain a record of animal(s)
sold one year from the date of sale. This information must be completed and attached with your EISF Livestock entry
form. You must provide this form for each species.
As an affidavit is deemed by USDA an an official record of Country of Origin. I attest throught first-hand knowledge,
normal business records, or producer affidavit(s) that all livestock referenced by this “bill of sale” are of
Origin. (list country in space, example: USA)
