INITIAL ASSESSMENT
Purpose: To assess mentee’s level of readiness in job search skills at the start of the mentoring
relationship.
Instructions:
a. Mentee completes this form first as a self- assessment.
b. Mentor and mentee re-visit throughout the relationship. Review regularly as you work
together to achieve goals and make progress.
Use the following descriptors for the rating scale in the tables:
1. I feel I need help with this
2. I feel this area is okay, but I would like extra support.
3. I feel this area is okay.
4. I feel I do well at this.
5. I feel I excel at this.
