Youth Scholarship Award
2015 Application
Purpose
The Lincoln Middle School Parent-Teacher Organization is offering a $500 scholarship to
all eligible Kenosha Unified School District High School seniors pursuing their education
at a two-year of four-year institution. The goal of the scholarship is to embrace the
pursuit of academic excellence and leadership potential by increasing access to
educational opportunities for those students who represent the cultural diversity of the
Greater Kenosha Community.
Eligibility
To be eligible for the 2015 Lincoln Middle School PTO Scholarship Award students must:

have attended Lincoln Middle School for a minimum of two years

have attended one of the following high schools: Mary D. Bradford, Tremper,
Reuther Central, Indian Trail Academy, LakeView Technology Academy, Kenosha
eSchool, or Harborside Academy

have demonstrated leadership through participation in school and community
extra-curricular activities

provide evidence of acceptance into an accredited college, university, or technical
institution at the time of the award (awards will not be granted to applicants
without this notification)

complete an application available at each school’s guidance office, Lincoln Middle
School, or by visiting http://lincoln.kusd.edu/

return completed application to Lincoln Middle School by Friday May 1st, 2015
