Fall 2013 Service to Success Scholarship
The City Colleges of Chicago (CCC) Service to Success Scholarship is awarded in
recognition of and in thanks for an individual’s military service. Each college will award up
to two scholarships to Chicago resident veterans and active duty military personnel. These funds
can be used to pay for any expenses to support his/her academic endeavors. Both full- and parttime students are welcome to apply.
Award Amounts:
Up to $500 per semester for students taking 12 or more credit hours;
Up to $250 per semester for student taking 6 – 11credit hours.
Eligibility related to military service (Confirm by form DD214 or orders):
Current active duty military personnel or those discharged from any branch of the U.S. military
for any characterization other than “dishonorable.” PLEASE attach a copy of your DD214 or
your orders with your application.
Academic criteria – a student must:
 Be enrolled in a degree or certificate program at CCC;
 Not previously have completed a Bachelor’s, or an Associate’s Degree;
 Have a minimum cumulative GPA of 3.0;
 Be enrolled in at least 6 credit hours required for the degree or certificate listed in the
student’s educational plan.
Deadline: The deadline for the Fall 2013 Semester is Friday, September 20, 2013 (5:00 pm).
Submission: Applications should be submitted to the Financial Aid Director at your home
campus by the deadline above. Completed applications (including the application form, your
Personal Statement, any required transcript(s), an Education Plan (if available) and a copy of your
DD214 or orders) should be placed in a sealed envelope with CCC Service to Success
Scholarship Application and your full name written on the front.

Please write a personal statement describing why you are seeking scholarship support. Use the
four statements below to help craft your essay. You do not have to address the statements in the
order listed; feel free to incorporate them in a way that will help the Scholarship Committee better
understand you and your strengths.

 Your commitment to academics, to your community and/or your involvement in
school/college activities;
 Your goals, ambitions and enthusiasm for your career field;
 Your financial need and how receiving a scholarship will impact you.
Your personal statement must be 150 – 500 words in length. It should be typed on 8 1/2" x 11"
paper; it should be double-spaced in 12 point font and have 1 inch margins all around. Please
make sure your full name is on the paper. Spelling, grammar and essay organization do count.
