Overview of CCG
NHS High Weald Lewes Havens CCG is a membership organisation that commissions
health care for a population of approximately 164,000 people and has delegated
commissioning responsibilities for primary Care. Its membership comprises 125 GPs,
working in 21 practices, every local GP practice is a member of the CCG, and has a
say in how healthcare services are provided for their local patients and community.
This principle of subsidiarity is part of the raison d’etre for the CCG, based on the
premise that GP members are closest to the patients and therefore best placed to
make decisions about what healthcare services our local communities need.
As part of the two year business plan first published in 2014, the CCG published The
Green Triangle – a manifesto for community services. This was made a firm
commitment by the CCG to radical change as follows.

Redesign of community services based on patient flows
Integration of Health and Social Care to deliver better responsive services for
patients
Implementation of a Primary Care Strategy that supports 7 day access and
provision of expert clinical advice; and GPs as gatekeepers to the whole
system of treatment and care.
Changes to Mental Health Services currently delivered in the community to
reflect local priorities and access needs.
The CCG has delivered on these commitments, successfully procuring community,
MSK, and diabetes services with more to come. It works in partnership with the two
other East Sussex CCGs and East Sussex Council, on a transformational programme
of change known as East Sussex Better Together (ESBT), providing a single
framework to co-design and reshape the Health and Social Care system, bringing
together the entire spectrum of services people need to be fully supported at every
stage of their health and care needs

The above aligns with the recently published NHS England Five Year Forward View
and East Sussex Better Together, supporting the expressed aims of NHSE with regard
to shifting the focus of care from acute settings to closer to home and the
development of new models of delivery. The Green Triangle is so embedded in
everyday CCG activity that it cannot be divorced from the four key priority areas and
East Sussex Better Together.
The rationale and focus of the role
“The Havens” is an area of relative deprivation within the CCG with poorer health
outcomes and significantly greater patient flows than average to acute services via
accident and emergency and the unscheduled care pathway. A programme of work
known as ‘Healthy Havens’ is underway to develop initiatives and services to
improve health outcomes, eliminate health inequalities, and build greater capacity
in primary care.
In addition a single handed General Practitioner is shortly to retire and it is
anticipated that the practice list will be dispersed and absorbed by the remaining
practices. These practices whilst very committed and well organised will face a
greater workload than they have already.
We therefore need to put extra resources into this area and think differently in order
to deliver the health outcomes we want to for this population. Hence the creation of
this exciting new role.
The role will encompass a broad mix of primary care medicine and will include
working with existing surgeries, both in clinic and home visiting. The post holder will
also work alongside our newly procured community health teams to provide
responsive and high quality primary and community care.
In the future we would expect the successful candidate to be involved , alongside the
CCG leads, in the development of a primary care hub / locality in this area perhaps
incorporating 8-8 working , dissolution of boundaries between in/ooh primary care
and working alongside and helping to develop the role of paramedic practitioners
and ANP’s in this model.
Overall Purpose of the Job:
The primary focus of this new role is threefold:
 First to provide additional capacity and support to four existing General
Practices in the Havens area (Newhaven and Peacehaven) by undertaking
sessional surgeries in the practices
 Second to participate in the rota to undertake home visits
 And to provide an urgent care service (still being designed), as part of our
plans to develop an extended hours urgent care facility for the local
population to minimise and avoid unnecessary A&E attendances and hospital
admissions. As part of this urgent care initiative you will be working alongside

the OOH multidisciplinary services to provide care of minor, moderate illness
and injury in the community.
Main Duties & Responsibilities
Clinical
In accordance with the timetable, and as agreed, to provide a range of general
practitioner services, to patients from four main GP practices namely; Meridian &
Rowe Avenue in Peacehaven and Quayside & Chapel Street in Newhaven, to:










Provide high quality personal medical services in the context of general
practice to the patients registered with the practice
To develop and maintain high levels of communication with patients and
carers ensuring effective care delivery
Provide clinical consultation and appropriate clinical management of patients
who attend the practices, including assessing the health care needs of
patients with undifferentiated and undiagnosed problems.
Provide home visit consultations and appropriate clinical management of
patients in their home and onward signposting / organisation of care as
required
Respond to medical problems presented by patients, including history taking,
physical examination, investigation, diagnosis, treatment and referral as
appropriate, as well as approaching on-going disease management
proactively.
Provide appropriate preventative health care, treatment and advice, regularly
screening for conditions found more commonly in vulnerable patient groups.
Provision of telephone consultation opportunities for patients who are
unable to attend the practice but do not require a home visit.
Ensure that timely, accurate and appropriate clinical records of all patient
contacts are maintained and updated on each point of contract. Good record
keeping will include participation in all appropriate manual and electronic
records.
Providing a patient centred service, ensuring that patient concerns,
comments and complaints are taken account of and dealt with in accordance
with the NHS Complaints Procedure.
Behaving at all times in a courteous and professional manner that encourages
high standards of quality and the embedding of good leadership and team
working with the wider multidisciplinary team.
Locality engagement, leadership, communication and governance
 To provide appropriate medical leadership as appropriate and promote
effective communication processes between all team members to enable
good working practices.
 To be actively involved in the Havens Locality and wider team meetings and
forums to help develop and shape future Primary Care service provision

Work as an effective locality team member and provide direction, leadership
and supervision to other members of the practice teams, including nursing
and administrative staff as required
To be actively involved in developmental processes within the four practices,
in partnership with other practices as appropriate
Develop positive collaborative working relationships across all areas of the
care team (primary, secondary and social care) and with carers.
Identify and utilise appropriately best practice through use of evidence,
benchmarking and clinical guidelines.
Participate in clinical governance activity and audit in order to maintain high
clinical standards and improve the quality of health outcomes for practice
registered patients.
Record clear and contemporaneous consultation notes to agreed standards
on the practice computer systems.
Undertaking administrative work arising directly from patient consultations,
i.e. referrals, letters, investigations etc. Ensuring team communication and
continuous monitoring of patient results, within agreed timescales, etc.
Ensuring the best possible standards of Infection Prevention and Control,
Health and Safety, safeguarding and data protection through compliance
with General Practice and CCG Policies
To participate fully with the Quality Outcomes Framework (QOF), working
with the individual practices and the CCG to ensure the best measurable
quality standards are achieved by the Practice for patients.
Training & Education





To provide appropriate health education and advice to patients and carers to
enable self-management and choice.
To undertake and complete appraisal process on yearly basis
To undertake appropriate training to meet personal and mandatory
educational needs and practice development needs
To undertake Practice/Personal development, in line with CPD objectives and
record activity.
To share and disseminate acquired knowledge with all practice team
members through in practice developmental sessions and clinical governance
meetings or critical incident reviews.
Additional Information:
Appraisal
To participate in individual performance review/appraisal for the purpose of
agreeing performance objectives and discussing development needs in line with
requirements of service need in the operational plan.
Clinical Governance
To have responsibility for a commitment to maintaining a high quality service to
patients by continual development of practice in the light of research evidence and
by audit, based against clinical relevant standards.
