Please include only edited and formatted text. Whatever is submitted in this form will be
posted to your profile. Feel free to leave sections blank if they do not apply or you do not
have data for them.

When you send this document, also attach a photo o be used on your profile. It
should be professional and clear of distractions in the background. The photo
should be at least 200x200 pixels. Larger is better (we can crop it) DO NOT embed
the photo in the Word document. Make it a separate attachment.
