REQUIREMENTS FOR RELATIONSHIP/PATERNITY LABORATORY SPECIMEN
COLLECTION
To help insure the accuracy and completeness of the enclosed paperwork, key areas of the forms
have been highlighted. Please have the adult(s) being tested fill in the appropriate yellow shaded
areas. The green shaded areas should be completed by the individual collecting the specimen(s).
1.
Chain of Custody Form:
All names should be filled in on this form.
2.
Consent Form:
Photocopy identification. Each individual must complete the entire form and present the
necessary identification to be photocopied (picture ID for adults and birth certificate for
children under the age of 18). Custody papers are necessary if the child is not in the
custody of the father or mother named on the birth certificate. Custody papers and picture
ID of Guardian should also be photocopied.
Photograph each individual. Each person must sign his/her own photograph except in
the case of minors. In this case the mother or legal guardian signs the child’s picture. A
witness must sign and date each photograph. If a Polaroid camera is not available,
enlarge the client’s picture ID so that the picture can be identified.
NOTE: In rare cases, such as home collections, pictures and copies of IDs/birth
certificates may not be possible. Witness must sign consent form and chain and attest
that identification has been verified.
MAKE SURE ALL APPLICABLE SIGNATURES ARE IN PLACE.

COLLECTING AND HANDLING BIOLOGICAL SPECIMENS
Please collect, air dry, label, place desiccant inside envelope, and seal the samples one individual at
a time!
1. Please use four swabs on each individual. (two per cheek)
Alleged Father : Blue Swabs
Child : White Swabs
Mother : Pink Swabs

Firmly scrape each swab against the inside of his/her cheek several times in each
direction, rolling the swab to coat with cells. Air dry the swabs for 5 minutes at room
temperature.

Place each person’s swabs in individually labeled envelope with packet of desiccant. Be sure the
label on the envelope is completed with the name of the individual whose swabs are packaged inside,
the date, and the collector’s initials.
4.
Use the red security tape (provided) to seal the envelope with the swabs and desiccant
inside.
5.
Fill out the Chain of Custody form with information on all specimens.

Adults & older children or cord draw:
Two 7ml EDTA (purple top tubes)
Young children:
Two 3ml EDTA (purple top tubes)
Label the tubes with the labels provided.
SHIPPING: SHIP WITHIN 24 HOURS OF COLLECTION (If unable to ship within 24 hours of
collection, store at 4o C). Packaging and transport are the responsibility of the collector. Test
participants may not package or transport samples. Samples not collected according to the above
procedure may not be accepted for testing.
