This document serves as a letter of request from a parent / guardian to the FGCLL Board of
Directors regarding the situation where their son / daughter attend a high school where
lacrosse is not offered as a sport. This also pertains to the situation where the student is home
schooled and the school the student is zoned for does not offer lacrosse as a sport. The student
must meet all the other eligibility criteria outlined in the bylaws to be considered. The request
is not granted until approved in writing by the FGCLL Board of Directors.

offered as sport, either through FHSAA or FGCLL. I am requesting that my (son/daughter) be
placed on a team in accordance with the process outlined in the FGCLL Bylaws. I am supplying
