If there is evidence of a
disability, referral for special
education evaluation is made
If there is NOT evidence of a
possible disability, a Prior Written
Notice is sent to parent indicating
refusal to evaluate
Parent sign permission to
evaluate form
Student’s progress is monitored in
general education. Parents may
access dispute resolution options
Evaluation is completed within
30 school days and an
eligibility decision is made
