Priority health area applications

Community Primary Prevention implementation applications

This section contains specific instructions on describing in narrative form the activities you propose to accomplish with a priority health area implementation grant.

Eliminating Health Disparities Initiative grants will be awarded to Priority Health

Area grantees to implement activities that will close the gap in the health status of populations of color and American Indians as compared to Whites in eight priority health areas.

In 10 pages or less, using 12-point font single or double space permitted, describe what you will do. a. What and who will you focus on?

 Whom are you trying to reach?

Briefly describe the population(s) you propose working with to improve their health status. Include in this section:

State the race or ethnicity of the population(s).

The age group(s) and gender of the people you propose to work with.

Information on local demographic of the population(s).

The health disparity as identified in Appendix C. If not identified as a health disparity in Appendix C, include data that documents a disparity.

You may attach documentation to this form. Attachment should not exceed one page, and will not count against your page limit.

What are you trying to change?

State which priority health area(s) you propose to address. Explain why you chose the priority health area(s). The eight priority health areas are:

 breast and cervical cancer,

 cardiovascular disease,

 diabetes,

HIV/AIDS and sexually transmitted diseases,

 immunizations for adults and children,

 infant mortality,

 teen pregnancy prevention, and

 violence and unintentional injuries.

b. What you will accomplish?

 What are your proposed activities?

Describe the strategies you are proposing to implement using Appendix D-

K; explain why you think it will achieve the objective you selected. If your proposed activity did not come from Appendix D-K, explain why you believe your proposed activity(ies) will achieve the selected objective.

Explain why you chose this activity and why you believe it will improve the health status of the racial/ethnic group(s) you have selected in the priority health area(s) you have selected.

Describe what the expected outcomes are for the proposed activities.

 Identify one or more measures that will indicate whether your activity had its desired outcome.

If your strategy will lead to a policy, system, or environmental change provide a description. A policy, system or environment changes can include distribution of resources, social support and community relationships, safe housing; access to transportation, health care, and nutritious foods; and, employment.

Attach a work plan that reflects your proposed activities, using the work plan template included in the Appendix N. Identify staff, facilities/locations and timeline you will employ in your proposed activities. The work plan does not count against your page limit.
