DIPLOMA IN TROPICAL MEDICINE & HYGIENE - REGISTRATION CHECKLIST 2012-13
Every participant must provide the following during the registration session:
Identification: Registration Staff must view your original passport
Copy of the passport cover page
Copy of the main passport photo page
Participants from outside the European Economic Area ADDITONALLY have to provide:
Copy of the relevant visa page
WE WILL NOT BE ABLE TO REGISTER STUDENTS WITHOUT APPROPRIATE ID.
WE WILL NOT BE ABLE TO REGISTER STUDENTS FROM OUTSIDE THE EUROPEAN ECONOMIC
AREA WITHOUT THE CORRECT VISA. PLEASE CHECK THAT YOU HAVE THE CORRECT VISA. YOU
WILL BE TURNED AWAY AT REGISTRATION IF YOU DO NOT HAVE THE CORRECT VISA. VISA

Please ensure that the following documentation, if not already submitted, is provided before or
during the registration session on 7th January 2013. Please email shortcourses@lshtm.ac.uk if
you are in any doubt regarding documentation required
Original documentary evidence of a medical qualification (unless you are GMC-registered or we
could verify your status as a physician with a current licence to practice online)
Original documentary evidence of registration for medical practice (unless you are GMC-registered
or we could verify your status as a physician with a current licence to practice online)
Original evidence of English test results if this was a condition of your offer and we have not
received the original test results as yet
