Kindergarten Central Enrolment Check List
My child has attended their 3 ½ year old Maternal Child Health check
My child will turn 4 years of age before 30 April in the year they will
attend kindergarten
I have chosen the Kindergarten I would like my child to attend
I have completed an online 4 year old kindergarten application form
OR
I have completed a 4 year old hard copy kindergarten application
I have attached a photocopy of my child's birth certificate and proof of
my residency
I have included the application fee
