THE ANN KRAFT SCHOLARSHIP IN NURSING
FROM THE NCHS LUMNI FOUNDATION
This scholarship in the amount of $350 and offered by the NCHS Alumni Foundation will be
awarded to a graduating senior who plans to pursue a degree in Nursing. The scholarship is
designed to support a student who intends to pursue studies leading to a career in any aspect of
nursing—CNA, LPN, RN, or Nurse Practitioner. While plans to enter the nursing field are the
focus of the scholarship eligibility, the applicant’s academic skills, commitment and service to
NCHS, service to the community, need, personal achievements, and goals will be taken into
consideration.
Eligibility
To be eligible for this scholarship the applicant MUST:






Be a 2016 graduate of NCHS
Have been accepted to a 2-year or 4-year college or university or to an
accredited nursing school
Be enrolled as a full-time student (12 semester hours or equivalent in quarter
hours or trimester hours in college credit-bearing classes) in the chosen school
and attend in the Fall of 2016
Be enrolled in a course of studies leading to licensure, certification, or postsecondary degree.
Have a current GPA of 2.5 out of 4.0 or higher
Submit the following as part of the application process
 This application form
 A copy of an official NCHS transcript
 A personal statement as noted in this application
 Two letters of recommendation, one from a current or former teacher
at NCHS, and one from someone acquainted with the applicant’s
academic abilities (this may be another teacher or counselor)
Consideration of applications will be confidential. Selection will be made by the Scholarship
Committee of the NCHS Alumni Foundation with the approval of the NCHS Alumni Board and
will be impartial, with all applicants receiving due and complete consideration. Application
materials will not be returned but will be kept by the Foundation for the period of one year before
being destroyed. Incomplete applications will not be considered.
Application Deadline: March 15, 2016, by the end of the school day
Submission of Materials:
All required materials should be placed in a large yellow envelope addressed to the NCHS
Alumni Foundation Scholarship Program and labeled indicating the scholarship for which you are
applying. The application must be submitted to the NCHS Counseling Office by the end of the
day on March 15, 2016. Late applications will not be considered.

Type your responses and attach to the Information Cover Sheet.
High School Activities: Clubs, Organizations, Athletics
List and describe briefly your level of participation in (leadership, offices held, participation in
projects, etc.) non-academic credit clubs, organizations, and/or athletics.
High School Honoraries:
List any honoraries to which you were invited and accepted membership. Briefly describe your
contributions to these honoraries by activities, offices, etc.
Volunteer Service to School and Community:
List any volunteer service in which you have been involved at NCHS or in the larger community.
Briefly describe the type and length of service.
Special Recognitions and Honors:
List any special honors, awards, or recognitions you have received while attending NCHS. These
may be within the school or the community.
Scholarships:
List the names, sources, and monetary amounts of any scholarships you have already received to
pursue your further education. Having received other scholarships will not preclude you from
consideration for this scholarship.
Personal Statement
This statement should not exceed two, double-spaced typed pages in 11 or 12-point font with
standard one-inch margins. Paper clip the finished statement to the Information Sheet before
submission of your application packet.
Topic:
In your best writing style, discuss (1) why you wish to pursue a career in nursing, and (2) how
you envision using your acquired skills to better the human condition.
Letters of Recommendation
You should request two letters of recommendation and include them in your application packet.
One must be from one of your current or former teachers at NCHS. The other should be from an
individual who can comment on your academic abilities, commitment to education, and personal
attributes that recommend you as a future nurse. Your second recommender may be another
teacher or counselor, if you choose.
Ask your references to address in their letters your academic abilities and your personal traits that
make likely your success in college and as a caregiver and health professional. These letters
must be confidential and be submitted in signed, sealed envelopes.
The recommender should address her/his letter to:

Note: This should be a personal letter, not a “To Whom It May Concern” generic letter.

Note: If you are applying for two of the scholarships, you need to provide only one copy of your

personal profiles, and personal statements. Please place these last three items in a separate yellow
envelope labeled with the name of the second scholarship for which you are applying.
