Malvern Aikido Club is affiliated to the British Aikido Federation (BAF).
The BAF is a founder member of the Joint AIkikai Council, officially recognised by the Aikido
Foundation Japan (Aikido World Headquarters).
Membership Forms
Membership is essential – this is to ensure that you and the people you practice with are
insured and can practice aikido safely. Therefore we ask that the two Forms below are
completed and returned to the Club Secretary or Instructor before undertaking any practice.
There are two forms to complete: the Application for Individual Membership
form is sent to the BAF for the processing of membership and associated insurance. The Club
Membership Details form is retained by Malvern Aikido locally. If you decide to apply for
Membership, you must supply a passport photo for your BAF Membership Book.
For new members, details of the arrangements for Temporary Membership is available on our
website malvern-aikido.org

Notes for completion of the Membership Forms
1. All data supplied will be used solely by officials of Malvern Aikido, the British Aikido Federation
and the Joint Aikikai Council for aikido-related business
2. Please write clearly (print in CAPITALS) and sign each form
3. ‘Medical conditions, disabilities or ailments’ refers to long-term situations that may affect your
ability to practice
4. Data Protection Act. Please read the declaration on each Form. You are entitled to see your
records by requesting them in writing from the Data Protection Officer of the BAF, the address
available from your Club Secretary or Instructor. Send a stamped, self-addressed envelope and
the BAF Data Protection Officer will respond to your request within 30 working days.
5. Malvern Aikido Club reserves the right to suspend or terminate membership without notice.
