Notes:
1. No radioactive waste to be placed in the hazardous waste store. Inform the Safety Officer of
any such material.
2. All substances in boxes must be identified, ‘Unknown’ is no longer acceptable. Please try to
identify e.g. organic/inorganic, solid/liquid, litres/kilograms.
3. Material in black plastic bags – complete contents listing is required e.g. gloves, rags, cloths
plus (and importantly) an approximate weight.
4. Material in blue plastic waste ‘bins e.g. glass, plastic, contaminated gloves etc. – the size of
the ‘bin and its approximate weight must be stated.
5. Where every item in a box is listed properly (use continuation sheet if required) there is no
need to include a size or weight.
6. Make 2 copies of the completed form – 1 with the consignment, 1 to Safety Officer, keep
original.
