Conditions Precedent for Execution of a Construction Contract:

a. Bond, and to support the Bond, a Certificate of Compliance issued by the

Ohio Department of Insurance, showing the Surety is licensed to business in

Ohio; b. Current Workers’ Compensation Certificate; c. Current Certificate of Compliance with Affirmative Action Programs, issued by the Ohio Department of Administrative Services, Equal Opportunity

Division under Section 9.47, ORC, by the State of Ohio Equal Opportunity

Center; d. Certificate of Insurance (ACORD 25 form is acceptable) and a copy of

Additional Insured or loss payee endorsement; e. A Certificate of Good Standing from the Secretary of State showing the right of the Bidder (if a foreign corporation) to do business in the State of Ohio; f. Proof of Current Licensing by the Ohio Department of Commerce,

Industrial Compliance Division, Ohio Contractors Industry Licensing Board

(OCILB), for all plumbing, electrical, hydronics, refrigeration, and heating, ventilating and air conditioning (HVAC) Contractors as required by

Section 4740, ORC; g. Evidence that the Bidder was enrolled in and in good standing in a

Drug-Free Workplace Program approved by the Ohio Bureau of Workers’

Compensation prior to submitting your company’s Bid for the Project; h. By submitting a Bid, the Bidder warrants that it is not now, and will not become subject to an unresolved finding for recovery under 9.24, Ohio

Revised Code, prior to the award of any Contract arising out of this

Project, without notifying the Office of the University Architect of such findings; i. EDGE Program supporting documentation - Evidence acceptable to the

Office of the University Architect of the Bidder’s participation in contracting with certified EDGE Business firms for the Project and/or your company’s good faith effort to contract with certified EDGE Business

Enterprise companies for this Project; and j. Declaration Regarding Material Assistance/non-Assistance to a Terrorist

Organization (DMA) - A properly executed form published by the Ohio

Department of Public Safety, to certify that the Bidder does not provide material assistance to any organization on the United States Department of

State’s terrorist exclusion list.
