B. Students who have graduated from high school but have not enrolled in any tertiary level school or vocational school.
C. Students who wish to transfer in Benguet State University.
1. Duly Accomplished Application Form (may be obtained from the BSU - Office of Student Services, Testing and Materials
Development Unit (Rm. 4), or from the Information Desk, Administration Building or may be downloaded from
www.bsu.edu.ph.
2. One (1) pc. ID picture (2x2 with white background and name tag attached to the application form).
3. Receipt of testing fee (PhP 200) from the BSU Cashier’s Office, Administration Building.
4. Certificate of enrollment from the High School Principal and two (2) photocopies of Form 137 (should include first year to
third year High School grades with General Weighted Average) duly signed by the School Principal or School Registrar for
currently enrolled high school students.
5. Two (2) photocopies of High School Form 138 (Report Card) for those who graduated from high school but have not
enrolled in any tertiary level school or vocational school.
6. Two (2) photocopies of the Certified True Copy of Grades for transferees.
Applications will be accepted from July 21, 2014 until November 28, 2014 from Monday to Friday, 8:00AM to 6:00PM
(NO NOONBREAK) at the BSU- Office of Student Services, Testing and Materials Development Unit Office (Room 4). Testing
dates will be given to the examinee upon submission of complete requirements.
