Revised 11/2014 Fayette County School System Dual Enrollment Contract and Advisement Form Part I: To be completed by student and parent/guardian Student Name ________________________________________________________ Home Phone ________________________________________________________ Cell Phone ________________________________________________________ E-mail address ________________________________________________________ College to attend ________________________________________________________ Program applying to: ______Accel _______MOWR Part II: To be discussed with the student and his/her parent/guardian in consultation with the high school counselor Student participation in the Accel program as defined in the Rule 160-4-2-.34 Dual and Joint Enrollment is a student’s choice. Because this choice exists for students, it is the responsibility of the student and the parent/guardian to be knowledgeable about the program and its positive and negative effects upon high school graduation. It is highly recommended that the student and his/her parent read and be familiar with Rule 160-42-2-.34 regarding the Dual Enrollment programs in Georgia as well as the Accel and MOWR regulations as posted on the Georgia Department of Education website. The information in this document is meant to supplement the Rule adopted by the GDOE. Accel Program: http://www.gadoe.org/ci_services.aspx?PageReq=CIServAccel MOWR Program: http://www.gadoe.org/ci_services.aspx?PageReq=CIServAccel The student and parent/guardian should be aware of the following: Attendance 1. Dual enrollment students who exceed three unexcused tardies per semester will conference with an administrator regarding McIntosh attendance expectations. Upon the fourth unexcused tardy per semester by a dual enrollment student, a letter will be sent home advising the parent of the dually enrolled student that the student's dual enrollment privileges will be revoked for the following semester should a seventh unexcused tardy occur. 2. Dual enrollment students who fail to follow established daily sign-in procedures after a warning from his/her guidance counselor will be subject to removal from the dual enrollment program the following semester. Revised 11/2014 3. On days where the high school schedule is altered, the high school schedule is always the first priority. Examples of this are as follows: PSAT Day, Milestones Test Days, Graduation Writing Test day and so forth. Students are notified in advance of these days and must communicate with their dual enrollment professors of their high school obligation. If this becomes a problem, then it is the responsibility of the student to notify his/her counselor immediately. 4. During the first week of school in August, all students are required to attend homeroom daily for the coverage of various materials and policies. The only exception to this would be if dual enrollment classes began during the same week. If you do not attend homeroom the first week of school, then the absences will count against exam exemption for the first semester. Course Credit 1. Students will earn one (1) Carnegie unit for each course offering (3) three or more semester hours successfully completed at the postsecondary level. For example: If a student is enrolled in a course worth 3 semester hours of credit that will equal 1.0 Carnegie units. 2. All courses taken at dual enrollment count as an academic course and will be averaged in the student’s HOPE GPA. Even if the course is taken for the purpose of an elective, it still counts in the student’s HOPE GPA with no exceptions. 3. A course may not be audited under a dual enrollment program. Full Time Enrollment In order for a student to be considered fulltime enrolled in high school he/she must take a total of 3 Carnegie units per semester. For example; taking one class at dual enrollment means you must take a minimum of four classes at your high school, taking two classes at dual enrollment means a minimum of two classes must be taken at your high school and so forth. Each student’s situation is different and it is very important for communication between the student and counselor to take place during the college registration process. During this communication, the counselor will ensure that the student is taking the appropriate courses in order to graduate. Grades 1. The high school will record on the student’s high school transcript each approved course name, grade and amount of credit earned for each course taken at the postsecondary level. Letter grades will be converted to numerical grades as follows: A = 95, B = 85, C = 75, D = 70, F = 50. Dual enrollment grades will not be weighted. 2. It is the responsibility of the postsecondary institution to distribute grades to its students. The high school will not issue report cards to students who are enrolled full-time in dual enrollment programs. Revised 11/2014 Course Incompletion or Failure 1. A student’s graduation may be delayed if a course needed for graduation is failed or if the student withdraws from a course needed for graduation. 2. If a student withdraws from a class or fails a class needed for graduation, he/she a. may not be able to enroll in a high school or college course that will allow him to graduate on schedule. In many high schools/colleges, courses are offered on a rotational basis; therefore, not every course is offered every semester. b. will re-enter the high school having missed the course content presented in the high school class(es) prior to his/her enrollment. This may hinder satisfactory progress in the high school class. Eligibility and Competitive Activities 1. Students enrolled in postsecondary courses must continue to meet the high school eligibility requirements for participation in high school competitive activities. Georgia High School Writing Test (GHSWT) Class of 2016 Only 1. The student is required to pass the GHSWT in order to receive a high school diploma. The GHSWT is based on Georgia Performance Standards (GPS). 2. The courses taken by the student in college will contain some but not all of the concepts and topics taught in the high school. NOTE: High school courses by law must be based on GPS. College courses do not have to meet this requirement; therefore, the student may experience some difficulty passing state assessments such as Milestones. 3. The student is responsible for making arrangements with his/her high school to take the GHSWT. Georgia Milestones Assessment 1. The student is required to take the Georgia Milestones Assessment if enrolled in a course that will earn credit for one of the following high school courses: Ninth Grade Literature, American Literature, Coordinate Algebra, Analytic Geometry, Accelerated Coordinate Algebra/Analytic Geometry, Accelerated Analytic Geometry/Advanced Algebra, Biology, Economics, and United States History. 2. The score earned on the Milestones Assessment will be averaged with the grade issued by the college. The Milestones Assessment will count as 20% of the final grade. 3. The courses taken by the student in college will contain some but not all of the concepts and topics taught in the equivalent high school courses. NOTE: High school courses by law must be based on GPS. College courses do not have to meet this requirement; therefore, the student may experience some difficulty on the Milestones Assessment. 4. The student is responsible for making arrangements with his/her high school to take the Georgia Milestones Assessment. The test must be taken on the day assigned by the high school and you will need to inform your dual enrollment classes of this schedule change. Revised 11/2014 Miscellaneous All dual enrollment students are required to join the high school Remind text notification group. The group text allows notifications of upcoming changes to the schedule or important information that needs to be provided to all dual enrolled students. The information on how to join this group will be provided once dual enrollment is confirmed and an official schedule is received by your high school. Each semester of enrollment in the dual enrollment program requires an ACCEL application to be completed. The application is how the college receives their funding and if it is not done then it is the student and parents responsibility to pay the tuition requirement. To create this account, each student must go to his/her www.gacollegee411.org account and create an ACCEL account which is located on the first box on the right hand side of the home page once you sign in. It is the student’s responsibility to create this account for each semester. Advanced Placement Courses Advanced Placement courses take priority over dual enrollment courses. Once the student signs the Advanced Placement contract committing to the AP course then the student must modify his/her dual enrollment schedule or choose to be full time enrolled at their high school. The master schedule for your high school may not be finalized prior to selecting dual enrollment courses and therefore a schedule change at the college may be necessary. An AP course will not be removed from a student’s schedule due to a conflict with dual enrollment Student Responsibilities It is the student’s responsibility to adhere to or complete the following: a. to follow all procedures and policies discussed in this contract, b. follow the admission procedures of the college, c. follow the rules and regulations of the college and the high school, d. communicate with the high school counselor routinely, e. check the high school’s website and newsletter periodically for information pertaining to the student, f. complete the ACCEL account for each semester of dual enrollment, g. receive advanced approval from the counselor to withdraw from a class or change colleges, h. arrange an appropriate high school/college schedule, i. provide accurate information (falsifying, misrepresenting, or knowingly furnishing false information may lead to removal from the program), j. follow his/her high school schedule daily and avoid altering his/her dual enrolled courses or exams to interfere with his/her high school schedule, and k. notify counselor immediately if a schedule conflict occurs between the high school and dual enrolled school Part III: Possible Classes for Dual Enrollment _____________________________________________________________________________ _____________________________________________________________________________ _____________________________________________________________________________ Revised 11/2014 Part IV: To be signed by the student and his/her parent/guardian We have read and understand all conditions and procedures outlined in the dual enrollment contract and advisement form. ______________________________________ ________________ Student’s signature Date ______________________________________________ ____________________ Parent’s/Guardian’s signature Date ______________________________________________ ____________________ Counselor’s signature Date
