The E-Lab enrollment form is an Excel spreadsheet. Please note it has multiple tabs/ pages. The notes below may be useful to you while as you are completing it. Please complete only the columns/cells documented below.
NOTE: This is a multi-purpose form, therefore a number of columns do NOT apply to E-Lab enrollment.

List users that will have access to Labs (Picasso EMR users that are not Providers)
Complete columns A through C for each user that should have access to Electronic Labs
Lab Information:

List Lab names & account numbers (if you use multiple labs, list all of them)

