Procedure for the re- issuing of a duplicate qualification statement of academic record
Information to applicant
1. An original qualification will not be re-issued to a student in cases where the original document
has been lost, damaged or destroyed. Only a duplicate qualification statement will be issued.
2. In the case of an original certificate being lost or destroyed, the applicant must submit an
affidavit ( a sworn statement that the certificate has been lost/stolen/destroyed, made at a
South African Police Services Station)
3. Compile a written application with the following information:
Surname
First names
Maiden name
Student number
ID Number
Telephone number
Tel number (w)
Tel number (home)
Cell number
E-mail address
Postal address
Indicate type of lost qualification: a degree/diploma or certificate
Last campus attended: Soshanguve/Ga-Rankuwa/Pretoria
Name of Qualification e.g. National Diploma: Nature Conservation

4. A fee of R85 per qualification and R100 per academic record prior 1988 must be paid into the
