We appreciate your selection of Associates in Family Care to meet
your individual or family’s health care needs. We strive to provide
quality medical care for our patients.
This pamphlet provides an introduction to our office policies and
procedures. The information is to let you know what to expect
from us and in return, our expectations of you. This is not a
contract, expressed or implied.
Family Medicine
Family medicine is a medical specialty which strives to take care
of the entire family through each stage of life.
All of our Doctors are board certified as diplomats by the
American Board of Family Practice and are trained to care for
people throughout all stages of life. They will do what they can to
help patients prevent disease whenever possible and to detect
disease in the earliest stages achievable so early therapy can
minimize the damage caused by the condition. When a medical
condition requires the care of a specialist, the Doctor(s) will make
appropriate referrals.

Office Hours
Our regular office hours are 8:00 a.m. to 12:00 p.m. and from 1:30
p.m. to 5:00 p.m. on weekdays, and Saturday mornings from 10:00
a.m. to 12 noon. The office will be closed on some holidays. For
after-hour emergencies, contact Memorial Hospital at (620) 2412250
Appointments
Appointments are necessary to see the Doctor(s). When acute
illness may occur, we request that you call as early in the day as
possible. If an appointment is required the staff can work your
visit into the schedule with minimal inconvenience to other
patients.
Since emergencies are unexpected, please understand that some
delays are unavoidable. We do take steps to stay on schedule, but
we will not compromise the care of a patient to do so. When we
know that we will be running significantly late, we will attempt to
contact patients to give you the option of rescheduling your
appointment.
Please call in advance to schedule routine appointments such as
physical exams (sports, camp, DOT, WCC, KBH, etc), paps, or
other non-emergency visits.
If it is necessary to cancel an appointment, we request 24-hour
advance notification, if at all possible, so that other patients may be
scheduled at that time. Continued missed appointments for
scheduled visits, without calling in advance to cancel, may lead to
dismissal from our practice.
Payment Policy
We accept cash, checks, money orders, debit and credit cards.
ALL COPAYS ARE REQUIRED AT THE TIME OF SERVICE.
Payments are required in full for private pay patients at the time of
service. In keeping with our commitment to serve our patients,
Associates in Family Care, LLC has secured arrangements for
partial payments for your convenience. If your bill is not paid in
full 30 days from the date of service or 30 days from the date the
balance is transferred from the insurance company you will be
required to enter into a payment agreement with Associates in
Family Care. The payment will be made in weekly/biweekly or
monthly installments.
The minimum payment amount is
determined by the total balance due. It will range from $30 to
$100. This will be determined upon signing of the payment
agreement. If you fail to promptly make the full payment when
due, your account may be sent to collections and/or the
patient/family may be dismissed.
Worker’s Compensation
If your visit is the result of an on-the-job accident, immediately,
inform the receptionist, nurse and/or physician. There are specific
legal requirements which must be met by the office in these cases.
These claims are mailed to the employer who must file them. Only
employment related problems may be dealt with at this specific
visit. Individual commercial health insurance will not cover onthe-job injuries. You are responsible for payments denied by your
employer.
Confidentiality
Medical information disclosed to us is considered private and
confidential. Our office is HIPAA compliant, which means
information will not be disclosed to any third party without your
written authorization, unless mandated by law.
Questions/Concerns
Any questions or concerns regarding a medical problem may be
communicated to the receptionist staff, which will be directed to a
nurse. Questions or concerns regarding insurance or billing may
be directed to the Office Manager. We are interested in any
feedback from you, our patient, in order to better serve you.
